First, Ulthera is a lifting procedure that uses high-intensity focused ultrasound (HIFU) energy.

It works by concentrating ultrasound energy into a single point and directing it to a specific spot in the skin as if firing an energy beam.
It does not affect the skin surface, but instead sends energy directly to the SMAS layer about 4.5 mm beneath the skin.

This fascial layer originally serves as a support structure that holds up the face, but as it ages, it stretches and loses elasticity, causing the skin to sag.
So, by using HIFU energy here to generate heat of 60–70°C, it creates thermal coagulation points, and during the body’s healing process, new collagen is produced.

As radiofrequency energy acts on the dermis, it generates heat, and this heat immediately contracts aging(?) collagen, then creates new collagen during the skin regeneration process.
So, even right after treatment, you can feel some tightening, but as time passes and the collagen regeneration process takes place, you can more clearly feel improvements in skin texture and tightening.

The main effects of Thermage can be described as improvement in fine wrinkles, reduction in pores, and skin tightening.
If Ulthera is strong in "facial contour lifting," Thermage is strong in "surface skin tightening."
It is effective for tightening and firming the skin overall.

So which one is better?
The answer to this question is: "There isn’t one!" There is a reason I can say that so confidently.
As explained above, the energy used, the skin layer it acts on, and even the intended effects are all different.

On the other hand, if the skin is thin and you are concerned about fine wrinkles, or if you are concerned about only mild loss of elasticity, Thermage may be a better fit.
