under-eye sagging does not happen from a single cause.
It appears as a result of changes occurring across
multiple layers at the same time, including the
skin, muscles, and supporting structures.
So even with the same treatment device,
results can differ depending on how it is approached.

Under-eye sagging does not have a single cause
To understand under-eye sagging,
you first need to know how this area is structured.
The under-eye area is not simply a single layer of skin.
The outermost layer is thin skin,
below that is the orbicularis oculi muscle,
and inside that is the orbital septum,
a membrane that supports the fat around the eyeball.

These three layers each weaken as people age.
The orbicularis oculi muscle also loosens over time,
and the orbital septum weakens and can no longer
support the fat inside it.
The problem is that these three changes do not
happen separately; they progress at the same time.

The skin sags downward, the muscle beneath it
loosens, and the membrane weakens so that fat
pushes forward—all at once.
As these changes accumulate, the entire under-eye
area starts to look heavy and saggy.
That is why it is important to examine what kinds
of changes have accumulated and then choose an approach accordingly.
- How does the Eye Ulthera effect appear?
Eye Ulthera delivers ultrasound energy in a focused way
to a desired point within the skin.
When the energy reaches the target area,
heat above 60°C is generated momentarily.
This heat causes collagen to contract immediately
and also stimulates new collagen production at the same time.
In the under-eye area, two different tip depths can be used.

The 1.5 mm tip stimulates the skin and dermal layer,
helping tighten the loosened surface,
and the 3.0 mm tip stimulates the deeper orbicularis oculi muscle and orbital septum.
In other words, it can act on the three-layer changes
described earlier at the same time.

There is a research paper on the Eye Ulthera effect,
so let’s look at it together.

This study measured, using CT imaging,
what changes Ulthera makes in the under-eye area.
After 12 weeks of observation, it reported that the weakened orbital septum had moved backward,
and the skin surface above it had also become flatter,
with statistically significant results.

The photo below shows the participant before and after treatment.

If you look at the left side before treatment,
the under-eye area is heavily sagging, and fine lines are visible on the skin surface.
On the right, 12 weeks later, the sagging area has been more organized,
and the surface texture also appears smoother.
Overall, the eye area looks much lighter and more defined.
What is noteworthy here is that the change did not happen in only one layer.
As the deeper membrane tightened, the protruding fat returned to place,
and at the same time, the surface skin was also refined,
creating a more balanced and lighter overall appearance.

What type of sagging is it effective for?
Taken together, Eye Ulthera is expected to show the clearest change in cases where
surface looseness and structural weakening in the deeper layers are progressing together.
However, when people are concerned about the under-eye area,
they often worry not only about sagging but also about fine lines.
In such cases, Eye Thermage and Eye Rejuran may be more suitable.

Eye Thermage delivers radiofrequency energy to the dermal layer,
helping refine skin texture and fine wrinkles.
If you have particularly noticeable fine wrinkles under the eyes,
Eye Thermage can help smooth the surface texture as well.
Meanwhile, Eye Rejuran directly injects a salmon-derived ingredient called PN into the dermal layer,
stimulating skin cell regeneration and helping restore the thickness of the dermal layer itself.
It is suitable when sagging is accompanied by dark circles or loss of collagen.
When the dermis has become thinner,
