How were they actually treated?
"I’d like my skin to look

This is something I often hear in the consultation room.
When I ask, "What concerns you the most?" many people answer that rather than pointing to one specific issue, they want to improve the overall untidy-looking impression of their skin.
The case I’m sharing today involves a man in his early 30s who visited a dermatology clinic for the first time at the recommendation of a friend.
Since he had several characteristics commonly seen in men’s skin, I think this case may be helpful for men who are concerned about their skin.
Let’s take a look at what his skin was like and how it was treated.
A Yeongdeungpo Man’s Blemishes and Enlarged Pores
Condition Before Treatment
To summarize his condition when he first visited us:

Brown blemishes were widely distributed as small spots across both cheeks and beside the nose.

Post-inflammatory hyperpigmentation (PIH), which remains where blemishes around the chin and mouth have subsided, was also visible.

The pores in the butterfly zone, extending from both sides of the nose toward the inner cheeks, were generally enlarged.

He normally produced a lot of sebum and had no prior experience with skin care.

On skin that produces a lot of sebum, blemishes are likely to leave red or brown marks after they appear and subside.
Also, when blemishes repeatedly develop in the same area, the pores gradually become stretched and enlarged. In this case, three concerns had accumulated at the same time: blemishes, enlarged pores, and blemish marks.
In particular, with regard to the pores, it was more important at this stage to prevent them from becoming further enlarged than to simply tighten pores that were already stretched. If sebum production remains unchanged, pores can become enlarged again even after treatment to reduce their appearance.
In other words, this was a case in which brown blemishes, blemish marks, enlarged butterfly-zone pores, and excessive sebum production all needed to be addressed together.
A Yeongdeungpo Man’s Blemishes and Enlarged Pores
Treatment Plan
When he first came in, he said, "I came to get toning." His friend had received toning and recommended it to him, so he had visited us thinking only about toning.
Toning is an effective treatment for lightening pigmentation. However, toning alone has limitations when it comes to addressing blemishes, pores, and blemish marks all at once.
Even when they look like the same type of pigmentation, brown blemishes on the surface layer of the skin and marks left after blemishes require different approaches.
To address the pores as well, the first step is to reduce sebum production before trying to tighten them. Therefore, rather than using simple toning, we planned a combination approach divided into stages for this case.

Brown blemishes and blemish marks were addressed with an epidermal laser.

A sebaceous-gland laser was used to reduce sebum production, helping prevent further pore enlargement and lower the possibility of recurring blemishes.

Already enlarged pores were treated progressively with a separate laser.
Rather than trying to resolve everything at once with a single treatment, we developed a plan to separate the different concerns and use appropriate lasers in stages.

Changes After Treatment
The treatment was carried out over a total of 10 sessions during six months. The combination of treatments was adjusted at each session according to the condition of his skin at that time.
To summarize the treatment by session:

During the first three sessions, we focused on reducing sebum production with the NeoBeam sebum laser, improving blemishes with Clarity, and improving blemish marks with collagen toning.

During the middle sessions, Fraxel, dot peel on the scarred areas, and Air Shine were used to improve deeper marks.

From the sessions after the midpoint onward, we focused on reducing the pores using pore lasers such as Sylfirm X.

Photo after treatment
The photo above shows his appearance after completing 10 sessions. The blemishes that had made his skin look untidy became lighter, and his overall skin tone and texture appeared more even.
In particular, blemish recurrence had almost stopped, and he said that not only he but also people around him often commented, "Your skin looks cleaner and better."
