Story Before Surgery
A female patient in her 30s came in saying, “My double eyelids are too large, so my eyes look tired, and my left eye doesn’t open well.”
In fact, she had very little eyelid fat and thin skin.
Because the left eye-opening strength was much weaker than the right, there were many overlapping folds, and she had what is called an Angelina Jolie-type multiple fold. Since she could not open her eyes properly, the double eyelids also would not fold properly.

Surgery Points
What this patient needed was not simply a “double eyelid surgery,” but
-
creating an appropriate double eyelid fold
-
reinforcing the strength of eye opening
-
releasing the overly high double eyelid
-
filling in the sunken eyelid
So we performed incisional ptosis correction + double-fold release + eyelid fat grafting (harvested from the abdomen), which were the procedures needed for this patient.
Eastern and Western eyes have different structures, and the adhered eyelids must all be released (blue line),
while adding fat down to the lower area (green line) and then re-establishing the double eyelid fold (red line).

That is why I think double-fold release is the ultimate form of double eyelid surgery.
There are many things to consider, and especially if severe adhesions remain from a previous surgery, or if a lot of muscle or fat has been lost, it becomes very difficult to lower the line.
But because I specialize in double-fold release, I feel a great sense of satisfaction when the line is lowered effectively.

After the surgery, opening the eyes became much easier, and the overly large double eyelid also stabilized into an appropriate size.
Thanks to the fat grafting, the hollowed-out look disappeared, and overall the eyes changed to look younger and softer.
There is still a little swelling in the front part of the double eyelid, but it will become more natural with time.
In the photo, the left eye looks as if it opens wider, but when speaking comfortably in real life, the symmetry is better.

In cases like this, patients often come in saying, “My double eyelids are too big,” but in reality, the cause is often asymmetry in eye-opening strength + lack of fat.
The problem is not something that can be solved by simply lowering the double eyelid fold; it is important to make the eyes open properly and restore the inner structures to their original state.
So it is difficult to solve this with simple line correction alone, and ptosis correction and fat grafting need to be done together for a natural balance.
In the case of double-fold release,

People who had double eyelid surgery but the size is too large

People whose double eyelids became too deep after surgery, making the surgery look obvious

People whose fold was placed too high, creating a large front fold and a small back fold that makes the eyes look older
it can be a good solution.
I think plastic surgery is not simply about ‘making things bigger,’ but about restoring balance.
This case was ultimately the result of balancing function and aesthetics.
