Q. I have a question about nose surgery: can columella lengthening and alar reduction be done together?
The first photo is my nose. If I do columella lengthening and alar reduction together, could the tip be made sharp instead of round?
And I’m also wondering whether it would make it look smaller.
I had ear cartilage harvested before in 3 or 4 places? Would it be possible to use ear cartilage for columella lengthening?
I’ve had nose surgery four times so far, but it still hasn’t improved, so I’m really upset...
Even if it doesn’t turn out exactly like the nose in the second photo, I’d like it to come out similar. Is improvement possible?
From your question, it seems that even after four surgeries, the nose shape you wanted still hasn’t been achieved, so you asked out of frustration.
It is difficult to judge based only on the photos you posted.
We would need to examine it in person to know for sure, but from the photos you sent, it seems that columella lengthening and alar reduction could achieve to some extent the shape you want.
However, if you have already had ear cartilage harvested several times, it may be difficult to use ear cartilage again for surgery.
From the photos alone, it looks like the nasal bone may need to be osteotomized and brought inward to create a slimmer appearance.
