This time, I’d like to introduce a revision case using autologous rib cartilage.
The patient had surgery 5 to 6 years ago using autologous rib cartilage and silicone.
They wanted a more glamorous and higher line with autologous rib cartilage, but
after the first surgery, it felt lower than expected and not as high as they had hoped,
so they returned to our hospital.



They also had a complex about their nose looking low and their mouth looking protruded.
Although it was not a protruding mouth, the nasolabial angle was unusually acute,
so it can be judged that the nose did not look as high and the mouth appeared more protruded by comparison.


Also, the forehead was quite high,
and compared with the forehead line, the bridge and tip of the nose felt a little insufficient.
Since the patient also wanted to maintain a smooth nose line starting from the forehead, we designed it based on the forehead.



Also, although autologous rib cartilage had been harvested before, because the patient wanted the nose to be higher, the amount from the existing harvest was not enough, so new cartilage had to be harvested.
In addition, alar base reduction was performed to correct the feeling of the nostrils being visible from the side and appearing spread out.



Looking at the results after surgery, the angle of the nasolabial angle has clearly come forward, making the mouth look less protruded and creating the effect of a prettier jawline.
Also, once sufficient height was achieved from the forehead all the way to the tip of the nose, the overall balance of the side profile improved, and the surgery was completed.
