Non-incisional alar reduction is a surgery performed using threads, and it is a procedure that relies on the strength of the threads.
Because of that, if you smile broadly or if the threads are pulled, there is a possibility that they may become slightly loosened.
For that reason, if there is absolutely no change between the alar width before surgery and after surgery, it is possible that the threads have stretched.
However, depending on how the alar width is measured, the width may sometimes appear different, but in your case, I think it is somewhat more likely that this is the reason.
The reason is that if the threads had loosened and returned to their original state, you should not feel pain in the alar area when smiling, and the philtrum would not protrude.
Since the threads have not stretched yet, they are still holding you in a way that prevents smiling naturally, and that is why the philtrum appears protruded.
As mentioned above, non-incisional alar reduction is a procedure in which the area is pulled by threads, so it is something that can feel uncomfortable for several days.
After about 1 week to 10 days, smiling should feel more natural, and you should be able to go about your daily life more comfortably.
