Hello. I am Kim Hyeong-taek, CEO of Kowon Plastic Surgery, who puts his full heart into nose surgery.
Today’s question is from someone who underwent revision nose surgery with autologous dermis-fat grafting because of symptoms of cartilage visibility, but the issue still has not been resolved.

Q. Revision surgery for cartilage visibility — I had dermis-fat grafting, but it’s visible again.
After nose surgery, I noticed signs of cartilage visibility, so I had revision nose surgery about a year ago.
For the revision surgery, autologous dermis-fat was grafted.
The cartilage was simply removed entirely.
But recently, the area where the cartilage visibility had appeared is showing up white again..
Can this happen even after autologous dermis-fat grafting?
There isn’t any cartilage left now, so I can’t tell why this is happening..
It’s not very visible in the photo, but in person it stands out clearly..ㅠㅠㅠ

As you mentioned, it is difficult to confirm cartilage visibility in the photo.
The photo would need to be a bit clearer for me to give a definite answer.
However, the area you mentioned appears to be slightly sunken compared to the other side.
It seems that although dermis-fat was grafted, it was absorbed and became depressed, which may have caused the cartilage underneath the nose to become visible.
Given your current situation, one possible way to address it would be to graft dermis-fat on the right side or use ear cartilage to fill the sunken area so that cartilage visibility does not occur.
For more details, please have a thorough consultation with the doctor who performed your surgery.
I hope you get a good result.
Thank you.
