Hello. I am Kim Hyung-taek, the CEO surgeon of Kowon Plastic Surgery Clinic, who puts his full dedication into rhinoplasty.
Today, I received a question from someone who had nose tip and nasal bridge surgery about a year ago and is now feeling stressed because their nasal tip seems to have drooped. They asked whether revision rhinoplasty is the only way to improve a drooping nasal tip.

Q. After surgery, my nasal tip has drooped. Is revision rhinoplasty the only way to improve it?
It has been a little less than a year since I had nose surgery.
Even before the surgery, my nose was not low, and after the surgery both the nasal tip and the bridge became very high.
That was actually what I was dissatisfied with, but now... I have no complaints about the bridge.
To be precise, I think I have not really thought about it because the drooping tip has been so bothersome. ㅠㅠ
At some point, I found myself unconsciously looking at the tip of my nose, and gradually I began to feel that the tip was drooping.
I thought maybe it was because my nose has always been relatively long, and people around me also told me that I was being overly sensitive.
But now they say it seems like my nasal tip really has drooped..
I can’t say that I always used cotton swabs when cleaning my nose after surgery, but I did use them most of the time.
If I didn’t have cotton swabs, I would roll up tissue paper and use that. I was careful and didn’t handle my nose roughly... sigh..
I am thinking about revision rhinoplasty, and I’m planning to go back to the hospital where I had the first surgery. What do you think?
Everyone talks about revision rhinoplasty, but... would I have to pay the full cost myself?
I attached photos, but since I covered my face, they may not be easy to see..
Please judge as much as you can from what is visible. (For reference, the photos were taken in the early 3 to 4 months after surgery, and they have absolutely not been photoshopped.)
It seems like the cartilage that should have been a little higher has moved slightly downward.. Why is this happening?
And is revision rhinoplasty the only way to improve a drooping nasal tip?
What would they say if I go to the hospital that performed the surgery?
If I do revision rhinoplasty, would it only involve the nasal tip? What kind of surgery would it be?
Could this be my fault? My younger sibling and my friend all had the surgery together, and I’m the only one like this..
I was the most careful, so I feel a bit wronged..

First, I think it would be best to talk in detail with the doctor who performed the surgery.
It is difficult to give an exact answer based on photos alone, so the first priority would be to compare the pre- and post-operative photos with the doctor who performed the surgery and check whether the nasal tip has actually drooped.
And if there really is nasal tip drooping, the drooped nose will not rise again unless revision rhinoplasty is performed.
If there is no major problem with the bridge and you are satisfied with it, then I think a surgery to raise only the nasal tip again would be appropriate.
Looking at the photos you attached, your lips are slightly protruded and you are smiling, so it is possible that the depressor septi nasi muscle is acting and pulling the nasal tip downward.
I can only give a more detailed explanation after examining the nose in person and accurately.
Therefore, it seems advisable to discuss this in detail with the doctor who performed the surgery.
I hope you get a good result.
Thank you.
