Q. Revision rhinoplasty for the nasal tip: what can be done about a bulbous nasal tip that turns into an arrow nose when smiling?
When I take photos, my nasal tip doesn’t look good like this.
And even with just a slight smile, it turns into an arrow nose.
What stresses me out the most is that it looks like an arrow nose and that the nasal tip is bulbous.
For my first surgery, silicone was used for the bridge, and only septal cartilage was used for the nasal tip.
These days, it seems like surgery is done using both septal cartilage and ear cartilage, so is my nose shape like this because only septal cartilage was used?
I’m looking into revision surgery for the nasal tip, and I’d like to know whether only the tip can be revised, and what process would be needed for revision rhinoplasty so that the nasal tip looks elevated and the line looks smooth from any angle, like in the photo below.
And I’d also like to know the difference between the person who had successful surgery and my nose.

Is your nose originally a little hooked?
The reason the nose becomes an arrow nose when smiling is a muscle called the depressor septi nasi at the nasal tip.
If this muscle is the problem, then unless it is cut, the nose will droop every time you smile and appear like an arrow nose.
In conclusion, the nose does not droop simply because only septal cartilage was used; it is a muscle issue.
At any clinic, it is possible to perform surgery on only the nasal tip.
Based on the current photos, to achieve a look like the photo below, it seems necessary to slightly lift the nasal tip more, reduce the bulbousness, and perform revision surgery that also changes the shape of the nostrils.

This is a revision case using donor rib cartilage with autologous rib cartilage.
The previously unsharpened hump on the bridge was refined once again, and the nose surgery was performed using osteotomy and rib particles.
Along with the effect of improving the nasolabial angle, the hollow area in the middle of the nasal bridge disappeared.

This is a case of revision surgery for a short nose.
The nasal tip was extremely short and turned up, making the face look awkward.
