Q. Last year, I had nose surgery with 3mm silicone + ear cartilage.

So recently, I’ve been seriously considering nose revision surgery again.
If I undergo nose revision surgery,
can the ear cartilage that was used before be reused as it is?
At the time, I was told that the nasal tip had been lowered as much as possible,
but in my eyes now, I still want the tip to come down a little more.
I’m wondering whether it is possible to lower it further from the current state.
To achieve the feel of a straight-nose surgery from my current nose condition,

→ This can only be judged accurately after seeing and examining the actual nose directly.
The possibility of straight-nose surgery should be assessed by considering the flexibility of the soft tissue at the tip, skin thickness, support structure, and related factors.
- What kind of surgery is needed to make it into a straight nose from the current state? Is it enough to just add silicone?
→ Since I have not seen your nose directly, I cannot say for certain,
but simply adding more silicone alone does not complete the straight-nose surgery you want.
A stable result requires considering several factors together, including tip support, cartilage arrangement, and skin condition.
- It seems the straight line does not appear because there is a bit of extra tissue at the nasal tip. Is correction possible with fat-dissolving injections?
→ In theory, it is possible to try fat-dissolving injections at the nasal tip,
but because the tip tissues become more sensitive after nose surgery, this must be approached very carefully.
If injections are done too aggressively, the shape may become distorted or tissue damage may occur, so caution is needed.
- Can I still get nose filler after nose surgery?
→ Nose filler augmentation is possible, but it must be decided very carefully.
If filler is added when implants or cartilage are already in place,
rare complications such as skin necrosis or blocked blood flow may occur, so you must consult carefully with an experienced medical professional.
For people like the questioner who are disappointed with the shape after surgery or need additional correction,

From the side as well, a noticeable change could be confirmed.
Fortunately, the saddle nose concern that the patient had worried about before surgery did not occur,
and they expressed satisfaction that the surgical result came closer to the image they wanted.
In particular, before surgery, the nose tip looked round and overall thick,
but after the procedure, it changed to a sharper and more elongated impression, giving a more refined look.
Also, before, the nasolabial angle was narrow, making the mouth look protruded,
but through surgery, that angle was adjusted naturally, improving the features into a harmonious and three-dimensional appearance.

Since this was a nose revision case, sensitivity to postoperative pain was high,
so, at the patient’s request, the surgery was performed under sleep anesthesia.
Also, because there was a possibility that the amount of cartilage needed might be insufficient, we informed the patient in advance that additional ear cartilage could be harvested before surgery, and proceeded after obtaining consent.
To improve the overall impression from stuffy to sharp,
we used about 3–4 mm of silicone to create a natural and harmonious line.

This case is a revision surgery case that included narrowing the bridge of the nose,
