This case shows how the structure of the nasal tip changes through the before-and-after process of bulbous nose surgery. This case had a bulbous nose shape that made the tip look broadly spread and gave a dull impression, with the nose overall appearing as though it was somewhat buried and stuffy.

In this kind of structure, the process of centering the nasal tip is more important than simply adjusting the height, and this part appears as the key change in the before-and-after comparison as well. While organizing the spread-out shape, some of the soft tissue was reduced and the widely separated cartilage was brought together to refine the shape, and during the surgery the thickness and direction of the nasal tip were adjusted together. In addition, a cartilage support structure was formed to reduce the tendency to spread again and create a stable structure, and the volume of the nasal tip was refined to create a slimmer and more balanced line.

The flow was considered so that the bridge and the nasal tip would connect naturally, and by balancing the columella, nasolabial angle, and philtrum line together, it was also possible to confirm an effect that made the mouth look more recessed. The relieved stuffy impression from both the front and the side, along with the overall balance being refined, is a characteristic visible in the before-and-after results of bulbous nose surgery.

Immediately after surgery is the stage before swelling has fully set in, so it is a point at which the formed nose line and structure can be compared relatively clearly when looking at the before-and-after results of bulbous nose surgery. However, in reality, changes are made based on the individual’s structural conditions, so rather than becoming a completely different shape, it is common to refine it in a harmonious direction.

In the case of a bulbous nose, the skin is often thick and there is a lot of soft tissue, so there are limits to changing it into a very small and thin shape. Looking at the appearance before surgery, you can see the features of thick skin and volume concentrated around the nasal tip.

In the view from a 45-degree angle after surgery, you can see that the dull feeling of the nasal tip has been refined and changed into a clearer shape. However, since this was about two weeks after the procedure, traces are still remaining in the area where the nostrils were refined.

Overall, you can see that the size of the nose has been refined and a slimmer impression has been formed. However, compared with a case that originally had a very small nose shape, it is difficult to produce the same level of change.

Also, if an overly small shape is created by force, the balance with the face may look awkward, so it is important for the changes to stay within a natural range. Adjusting according to the original conditions and facial proportions helps create overall harmony.
