In fact, with this type of nose, simply raising it is not enough to achieve the desired look. The width and direction of the tip, the smooth connection with the bridge, and the overall volume of the bulbous nasal tip all need to be addressed together to create a harmonious and refined result.

These days, many people prefer a “natural-yet-glamorous nasal line” that is both natural and striking. However, if the nose is raised indiscriminately when the face lacks overall dimension, the nose alone may look as though it is floating. That is why I focused on balancing the entire face rather than making the nose excessively high.

[Surgical Plan and Method]

After analyzing the proportions of the entire face, I appropriately augmented the nasal bridge with silicone and made careful adjustments to bring out clear definition in the center of the face. For bulbous nasal tip surgery to refine the spread-out nostril wings, the cartilage was repositioned and secured, while unnecessary soft tissue was neatly removed. A firm autologous cartilage support was also placed to improve stability and help prevent the shape from becoming distorted over time.

Rather than simply narrowing the width, I focused on creating a smooth curve connecting the bridge and the tip. To reduce the heaviness characteristic of cases with thick skin, I combined delicate tissue removal with bulbous nasal tip surgery to create a more refined line.

I designed the nose around a current trend: a line that is not excessive but still brings out refined details. Adding dimension to the face can help create a more polished overall impression. In this case, the nasal tip was short and upturned, making the nostrils somewhat clearly visible from the front. The nose itself was relatively small, and there was not an ample amount of septal cartilage available for use during surgery. Therefore, the key was to extend the nasal tip sufficiently and with enough length.

To supplement the limited available material, we used ear cartilage or rib cartilage to secure a sufficient amount of graft material. We then focused on clearly extending the nasal tip and columella forward to balance the entire face. During this process, the nostril wings, which had been recessed toward the cheeks, were brought out more clearly, while the blunt, bulbous nasal line was also smoothly refined.

Although the nose may appear simply short and round at first glance, I combined delicate refinement with nasal tip lengthening and designed it so that the bridge and tip would connect harmoniously.

Even though swelling was still present immediately after surgery, the previous round and somewhat unsophisticated appearance was neatly refined. From every angle, the nasal contour took on a refined, feminine, natural-yet-glamorous appearance. The mouth area, which had appeared more prominent, was also naturally balanced, completing a more polished overall impression.
