I am Director Lee Won of 21 Plastic Surgery.
Today, through an actual patient case,
I’d like to share the three types of facial contouring surgery
and a facial contouring review.
Contour surgery can be divided into square jaw surgery, cheekbone reduction surgery,
and chin tip surgery.
These three procedures are often chosen by people who want to refine the overall facial line,
but depending on an individual’s skeletal structure,
some people undergo only one or two of them.
In the past, contour surgery was often viewed negatively
because it involves shaving bone.
However, with advances in modern medical technology
and growing interest in appearance,
more and more people are visiting hospitals after reading facial contouring reviews.
But contour surgery should not be decided based on
facial contouring reviews alone!!!
Since surgery involves working with bone,
accurate diagnosis and planning are more important than anything else.
Unlike other parts of the body, once bone is damaged
and removed, it does not regenerate,
so if excessive resection is performed without considering
my facial skeleton or bone thickness at all,
the face shape can become unnatural,
and side effects such as cheek sagging may occur...
So what should you do??
This is the most important part.
For a satisfying result,
it is important to accurately consider your own skeletal structure and bone thickness
and find the optimal balance.

Front/side before surgery
The patient visited our hospital after struggling with a wide, flat face shape.
Usually, people with a bumpy facial contour line undergo contour surgery more often,
but even if the facial contour line is not bumpy,
people whose faces look wide or flat also often undergo contour surgery.^^
This patient was also such a case,
and wanted a face with a more pronounced frontal effect overall
and an oval face shape.
The chin tip was not particularly long,
but since the patient wanted it to look slightly shorter,
we planned it so that the chin tip would appear shorter and slimmer,
with a feel somewhere between a natural U-line and a glamorous V-line.
These days, when most people come in for contour surgery,
they usually undergo only square jaw reduction surgery or cheekbone reduction surgery,
and do not often come in thinking specifically about the chin tip.
The reason I am saying this is that
in cases like this,
if the square jaw and cheekbones become slimmer and narrower
but the chin tip remains unchanged,
the overall facial balance can be disrupted,
and in some cases the face can appear longer^^;;
In such cases, I strongly recommend considering chin tip surgery as well.
Surgical plan
Cheekbone reduction surgery: Rotation & reduction (4mm/4mm) plate fixation
Square jaw surgery: Long curved resection (10mm), cortical bone rasping
Chin tip T-osteotomy reduction surgery: (6~7mm) width osteotomy
Fat grafting: forehead, nasolabial fold area

As you can see from the patient’s X-ray,
the left cheekbone is protruding higher.
Because there was a difference in bone height,
the amount of soft tissue was also different,
so we proceeded to achieve a certain degree of symmetry on both sides.

This is one week after surgery.
Even with swelling present one week after surgery,
you can clearly see that the facial line has become much slimmer.
Because the patient did not have much bleeding,
there was only a yellowish bruise instead of a dark bruise, and recovery was fast.
So the swelling and bruising will probably go down quickly too,
and recovery should be fast.

After one month, most of the swelling had subsided.
Recovery speed may vary depending on whether there was bleeding,
so sufficient rest and care are important.
If there was significant bleeding,
you should note that the recovery period may take more than 1 to 2 months.

If you look at the photo from the two-month mark,
all of the swelling has now gone down.
This is exactly the frontal effect of facial contouring.
The facial line has become smoother and more feminine than before.
It seems to have become the oval face shape the patient wanted.^^

If you compare the before-and-after X-rays,
the difference in volume between the cheekbones and the jaw is enormous.
The protruding cheekbones have been refined,
and you can see that the square jaw and lower jaw have also been smoothed out.
Today, together with an actual patient case,
we took time to look at a facial contouring review.
Bone surgery for the face requires not only a CT scan
but also an accurate diagnosis.
This is because if you receive the wrong diagnosis
or proceed with surgery without considering your facial proportions,
the facial balance can be disrupted
and the face can become awkward..
So no matter how good a facial contouring review may look,
be sure to visit several hospitals and make your decision,
and you should be able to achieve good results.
