The impression of a face is not determined simply by the skin or the shape of the eyes and nose.
Only when the overall facial structure is balanced
can a more harmonious facial shape be achieved.
Many people want a slim and three-dimensional face,
but since everyone is born with a different facial skeleton,
we should not perform surgery in the same way for everyone!!!
In modern society, appearance has become more than just an aesthetic factor;
it is an important element that also affects my confidence and social relationships.
In particular, for occupations that frequently show their faces to the public,
such as entertainers or influencers,
this becomes an even bigger concern.
Beyond that, as self-branding has become important even among ordinary people,
interest in appearance management has increased.
However, contouring surgery is not simply
about making the face smaller,
but a process of adjusting facial proportions
and completing the optimal line tailored to each person’s bone structure.
So, what is contouring surgery,
how is it performed,
and what is the before-and-after change process like?
Today, I’ll talk about this together with a real patient case.
The basics of bone surgery
are plain X-rays.
The rule is to look at the plain X-ray and then check the CT scan.
Especially for things like bone fixation or contour shape,
plain X-rays are often better for seeing the before-and-after of contouring surgery,
and they are also useful for identifying side effects^^
But the problem is that there are many cases where people do not look at the plain X-ray
and instead prioritize the CT scan!!!!!!!
Of course, it is fine to look at CT first, and in many cases it is more detailed,
but the issue is that people only look at the impressive-looking 3D images from the CT...
In other words, they evaluate everything
just by looking at the 3D before-and-after contour photos.
Even if a patient says
where it hurts and where it feels uncomfortable,
they judge it as normal based only on the external appearance.
Let’s look at the photos of the patient below.

Preoperative CT
If you look at the patient’s CT image, you can see that the bone is severely偏 toward one side.
In such cases, not only facial asymmetry
but also the aesthetic aspect when viewed externally
may be reduced.

Front/side before surgery
If you look at the preoperative photos, the volume in the front cheekbone area is insufficient,
and one side of the jaw is overly developed, making the face look long vertically and the line appear rough.
The cheekbones are developed in the side and 45-degree views,
and the angle of the square jaw is distinct, giving the face a angular appearance..
That is why the patient came to see me because of these issues.
Surgical plan
Zygoma reduction (including deep cheek fat repositioning)
rotation & reduction plate fixation
Second-angle rasping
Dissolving filler in the chin
Long-curve square jaw surgery
Angle resection cortical bone rasping
T-osteotomy reduction

As you can see from the patient’s X-ray,
on the X-ray, the left jawbone is seen to be
thickly developed.
Since there was a difference in both bone height and width,
the amount of soft tissue was also different,
so the procedure was performed to achieve symmetry on both sides.

Front/side after one week
It has been a week since the surgery,
but the entire face is still swollen due to the swelling.
Fortunately, this patient did not have a large amount of bleeding during surgery, so there was no bruising visible,
and only a yellowish bruise appeared normally.
So the bruising will probably go away quickly?
Recovery also seems likely to be fast.

Front/side after 3 months
As three months have passed, most of the swelling has gone down.
What you are seeing is the frontal effect of facial contouring.
When the face looks uneven to one side from the front,
the facial line becomes more defined,
so the previously blunt-looking face seems improved^^
Sometimes, for those who worry a lot about whether the before-and-after difference will be clear,
there are times when they request excessive removal.
To state the conclusion first,
excessive removal is a shortcut to ruining the face!!!!
If too much bone is removed,
the biggest problem is nerve damage.
Once bone is cut, it cannot be restored.
In particular, if a nerve is cut, it can cause nerve damage that can never recover.
Sometimes, there are people who say they do not mind losing sensation and ask to remove as much as possible,
or people who are thinking about removing a lot;
it would be good to know that this can lead to such terrible results.

Before surgery / One week after / Three months after
Today, we took time to learn about before-and-after contouring surgery with a real patient case.
Surgery that cuts facial bones requires not only a CT scan
but also an accurate diagnosis.
If you receive an incorrect diagnosis or undergo surgery without considering your facial proportions,
your facial balance can be ruined
and the face may become awkward.....
So no matter how effective the before-and-after results of contouring surgery may seem,
you must!!!!!
visit several hospitals and decide after comparing them,
and you are likely to get good results.
