Chin Surgery: Chin Advancement + Double Chin Muscle Tightening, Real Patient Review

This is Director Lee Won of 21 Plastic Surgery.

Today, we’ll look at a real patient case who underwent chin advancement and double chin muscle tightening,

and go through the patient’s review together.

The highlight of facial contour surgery can be said to be chin surgery.

For patients with severe microgenia,

the effect is so dramatic that they often say,

“Please do chin advancement surgery.”

For people whose chin is recessed or small,

the jawline looks flat...

A double chin appears even when there isn’t much fat...

The mouth looks relatively protruded...

In the end, these things become major sources of stress.

Patients with a recessed chin often also have a very developed double chin. ^^;

What can dramatically solve these problems is chin surgery and double chin surgery.

Preoperative frontal/lateral CT

Based on the patient’s preoperative CT images above,

the right side of the chin is more developed in the frontal view,

and in the lateral view,

the lack of chin projection makes the mouth look protruded.

As a result of CT image analysis,

we confirmed mandibular asymmetry on the left and right sides,

and, based on the Ricketts line,

the deficiency of the chin.

Also, if you look at the preoperative image,

you can see that the short chin makes the double chin stand out noticeably.

The problem is much more obvious from the side than from the front.^^;;

The patient said that they had been very stressed because the recessed chin made their overall impression look dull...

So, after much consideration, they decided to undergo chin advancement and double chin surgery.

Surgical plan

Chin advancement surgery (T osteotomy + advancement)

• Mini V line (secondary angle reduction)

Double chin + bulldog fat suction + muscle tightening

• skin excision + salivary gland tightening

As I always say,

before choosing a surgical method,

you must remember that an accurate diagnosis of your own condition

must come first!!!!!!!!

Preoperative / 45-degree postoperative

This is the patient’s progress at 1 week after surgery.

It has only been 1 week,

but with the chin slightly brought forward,

the double chin has improved a lot.

And because the patient had little bleeding,

they only developed yellowish bruising,

so I think the swelling and bruising will go down quickly and recovery will also be fast.^^

However, even though chin surgery solves so many issues,

do you know what question patients ask most often about chin advancement surgery?

It is the issue of the secondary angle.

As shown, after chin advancement surgery,

a secondary angle is inevitably created.

So through mini V line surgery,

we do not touch the angle under the ear,

and only cleanly remove the secondary angle.

The patient did not want the chin to look longer after chin advancement,

so we advanced it at an angle

to prevent the chin from becoming longer.

This design and method are not applied equally to all patients.

Please note that the patient’s requests and what is possible

are determined through multiple consultations.

Today,

as a method for treating microgenia,

we looked at chin advancement surgery and double chin muscle tightening

through the case of a real patient.

Chin advancement surgery uses the patient’s own bone,

which improves the overall proportions of the face,

makes the lip line look more defined,

and creates a more attractive change.

But if there are advantages, there are also disadvantages.

Compared with inserting an implant,

the recovery period can be longer,

and depending on the individual,

the change in chin volume may not appear as much as expected,

so you should make your decision only after fully understanding these points.

There is a rumor that if chin surgery is done with a T osteotomy,

bone resorption occurs and the chin dissolves,

but that is absolutely not true!!!!

There are various chin osteotomy methods,

including T osteotomy, Y osteotomy, ㅅ osteotomy, M osteotomy, W osteotomy, and more...

Among these, there are methods that should not be used,

and methods that are safe and appropriate to use.

That is why I hope you can undergo surgery with a proven and safe method with peace of mind.
