Facelift Surgery Without a Pulled Earlobe: A Real Patient Review

A facelift(Facelift) is

surgery that improves facial sagging

by restoring a face that has lost its firmness

and giving it a more youthful appearance again.

However, many patients considering this surgery

have a common concern.

That is the ‘pulled earlobe(Scarred or Pulled Earlobe)’

phenomenon.

Before surgery / after surgery, front view

A pulled earlobe refers to the phenomenon in which, after surgery, the skin around the ear is unnaturally pulled, causing the ear to become long and deformed, as if it were ‘pulled like a knife.’

This problem goes beyond a simple aesthetic issue

and leads to things like

“visible surgery scars,”

“tightness around the ear,”

and “unnatural side profile,” which in turn reduces patient satisfaction...

So, how is it possible to perform a facelift

that looks more natural

and also allows for faster recovery

without a pulled earlobe?

Today, I’ll explain the answer and show you a real patient case in detail^^

Before looking at the patient case,

there are 3 key points for a facelift without a pulled earlobe.

Facelift incision line

The first is the design of the incision line.

When making the incision, it must follow the ear’s curves.

In particular, when passing through the tragus,

the thickness and direction of the skin, as well as the anatomical structure of the ear, must be considered,

and the earlobe and the line beneath the ear

should be designed in a curved direction that allows the skin to flow, not in a direction that pulls it.

This approach can be the first key to preventing a pulled earlobe.

Facelift dissection area

The second is the SMAS fixation direction and tension control.

A lift that simply pulls the skin

not only does not last long,

but also places excessive tension on the skin itself,

which causes a pulled earlobe.

So when performing facial sagging surgery,

the SMAS layer must be dissected,

and the fixation direction of the SMAS should be designed away from the ear

so that almost no tension is applied,

and firm SMAS fixation should be used

to prevent the skin from being pulled.

This helps reduce the problem of pulled earlobes^^

The last is the suturing method.

Our ears are made up of very delicate structures.

In particular, the earlobe has few blood vessels,

so it is very sensitive to scarring or deformation!!!

That is why the earlobe and soft tissue

must be separated and settled into place,

then finished with very delicate suturing.

Now that we have reviewed the key points of a facelift,

let’s take a closer look at an actual patient case^^

Before surgery, front/side view

The patient above is a woman in her 60s from Japan.

She had previously undergone a facelift 7 years ago, but wanted to improve the loosened skin again

as well as the existing surgical scar.

Procedure

Deep plane facelift re + neck lift, PRP

1 month later, front/side view

This is the appearance one month after surgery.

From the front, the bulldog line and deep nasolabial folds have been smoothed out,

and the facial line looks more clearly defined^^

In the side view, because the procedure was performed while preserving the preoperative earlobe shape,

the earlobe is not pulled at all,

and there is no sign of a pulled earlobe phenomenon.

6 months later, front/side view

This is the appearance at 6 months.

Over time, there is a slight sagging around the nasolabial area,

but it has not fully returned to the original state,

and we were able to confirm that the postoperative result was being well maintained^^

Many people consider a facelift, but hesitate because of “obvious signs” such as surgery marks from facial sagging surgery or a pulled earlobe.

However, with accurate anatomical understanding, delicate surgical techniques, and follow-up care,

it is possible to achieve a result that is sufficiently natural

and highly refined without a pulled earlobe.
