Hello,
This is Director Lee Won of 21 Plastic Surgery.
Under-eye fat repositioning surgery may look like a simple procedure,
but in reality, it is a high-difficulty surgery that requires
precise anatomical understanding
and skilled technique.
Through this under-eye surgery, some people say,
"My eyes look clearer and my overall impression improved a lot,"
while others
complain, "I actually look more tired now, so I'm considering revision surgery..."
So why do the results differ so much
when the same under-eye surgery is performed?
The difference lies in anatomical diagnosis, surgical planning,
and the technique used to handle the fat.
In this post, I will explain in detail the key factors that determine
whether the result is satisfying or disappointing,
and when revision surgery may be needed.^^

Under-eye structure
The under-eye area is not simply a bulge made up of
fat alone;
rather, the following structural factors
work together in combination.

Orbicularis oculi muscle

Tear trough
If these structures are not identified accurately
and the fat is simply moved,
it can lead to hollowness, unevenness, shadowing, discoloration,
and a tired-looking appearance....

Now, let's look at an actual patient case.
This is a female patient in her late 20s.
She had under-eye fat repositioning surgery 5 to 6 years ago
and underwent aegyo-sal filler injections about twice,
but the effect was minimal and her dark circles became more severe,
so she came to our clinic.^^
From the patient's appearance,
the hollowness under the right eye was severe,
and the asymmetry between the left and right sides was pronounced.
Surgical plan
Through the conjunctiva, some of the previously repositioned fat was
partially removed and reorganized,
and the hollow areas were evenly filled with fat grafting
to make the surface smoother and less sunken.

1 month later, front/side view
This is the appearance after one month.
From the one-month mark,
the under-eye area became smooth and refined,
and we could confirm that the grafted fat had settled properly.^^
The patient was also very satisfied,
and said that after surgery she ended up wearing less makeup,
which made her very happy~^^

Finally, here is the patient's appearance two months later.
The fat on both sides settled well without over-resorption on just one side,
and the bluish-looking shadow under the eyes was reduced,
which made the patient's expression look brighter.^^
To briefly state today's conclusion,
the key to under-eye surgery without revision surgery is
the harmony of anatomy + technique + the medical team's experience!!!
Under-eye surgery can create a more youthful appearance,
but because the under-eye area is such a delicate region,
even a small technical error
can create a major difference in the result!!
At our clinic as well,
more and more people are coming in for revision consultations,
saying,
"I already had surgery at another clinic,
but I'm not satisfied,"
or "I actually look more tired and feel uncomfortable."
Looking at these cases,
it is often not simply a matter of lack of technique,
but rather that surgery was performed in a uniform way
without sufficiently analyzing each patient's under-eye structure,
skin condition,
and fat distribution...
