Hello.
I am Director Lee Won of 21 Plastic Surgery.
As interest in under-eye fat repositioning surgery has increased lately,
many people have been asking about swelling after surgery.
Among them, one of the most common questions is:
“Is it normal that the swelling under my eyes seems to last too long?”
Many people feel self-conscious in daily life because they look tired or down,
and they consider surgery to address the puffiness or hollowness under the eyes
that makeup cannot cover...
So today, together with an actual patient case,
I will explain in detail from a specialist’s perspective
why swelling can last a long time after under-eye fat repositioning.^^

Front/side view before surgery
The patient is a woman in her 40s.
Recently, she had often been told that she looked tired and worn out,
which lowered her confidence,
and she said she was under a great deal of stress because of the puffiness and hollowness under her eyes.
After checking her condition in the clinic,
it was found that the orbital septum had weakened,
allowing the fat to shift forward and become bulged!!!
Overall, the under-eye boundary was deeply sunken,
creating shadows that made the dark circles look even more prominent...
So the surgical plan for the patient was as follows:
Through a transconjunctival approach, the protruding fat was evenly spread out,
and the hollow areas were filled with autologous fat harvested from the patient,
so there would be no unevenness.
The key to this surgery is not simply filling only the hollow areas,
but smoothly connecting the overall volume flow and line of the under-eye area.^^

1-week follow-up front/side view
This is the 1-week follow-up.
At first glance, swelling and bruising are noticeable,
but this is a normal early-stage finding,
and if you also manage it mainly with cold compresses,
it can help swelling and bruising subside.^^

1-month follow-up front/side view
This is the 1-month follow-up.
The bruising has mostly faded,
but a slight amount of swelling remains.
However, the under-eye area that had looked hollow and protruded
has naturally settled in,
and the aegyo-sal line is also beginning to take shape naturally.^^

2-month follow-up front/side view
This is the final 2-month follow-up.
The visible bruising and swelling have all gone down,
and the under-eye volume can be seen to have filled in naturally!!!
As the aegyo-sal settled in naturally,
the tired-looking impression before surgery disappeared,
and with a small change, the face transformed into a brighter image.^^
Like this patient, even when the surgery itself is successful,
there are often cases where recovery appears relatively slow during the healing process.
Why does this happen?
The reason is mostly because it is based on anatomical and physiological factors!!!
Under-eye fat repositioning is a high-difficulty procedure that does not simply remove protruding fat,
but changes the impression by repositioning tissue.
Therefore, the recovery speed also varies from person to person,
and swelling between 2 weeks and several months can be regarded as a natural part of the recovery process.^^
Swelling itself is not dangerous;
in fact, it may be a sign that the body is adapting to its new state.
