Before-and-after rhinoplasty lip-philtrum surgery real review: actual changes from philtrum reduction + nostril lowering

Today, I’d like to explain philtrum reduction surgery, that is, lip-philtrum surgery and nostril lowering,
along with the patient’s actual progress photos.^^

Front view before surgery / front view after surgery
The philtrum itself does not have any special function like the eyes or nose,
but because it is located between the nose and lips,
it has a major effect on the facial image!!!
For example, if the philtrum is long,
it can make the space between the nose and lips look longer,
which may negatively affect facial balance.
And if the inner philtrum is indistinct,
the three-dimensionality of the lower face may look reduced,
or it may appear unnatural.
Also, if the nostrils are generally too visible,
they can give a somewhat unsophisticated impression...

Side view before surgery / side view after surgery
So for people whose nostrils are very visible,
many think, “Should I get alar reduction?”
But if the nostrils are very visible from the front,
correcting them through nostril lowering can produce good results.^^

The case I’m introducing today is a woman in her 30s.
Her philtrum was not only long and flat,
but the nostrils were tilted upward, so when viewed from the front,
the triangular nostrils were especially prominent.
After diagnosing the patient,
we determined that conventional philtrum reduction alone had limitations,
so we combined it with nostril lowering based on composite tissue grafting.

Front view / side view after one week
At one week after surgery, after stitch removal, swelling and bruising are at their peak.
If you look at the photos, you can clearly see that the philtrum has been reduced.
The nostrils also show downward volume when viewed from the front,
and the amount of visibility has been reduced compared to before surgery.^^

Front view / side view after 3 months
Three months after surgery,
most of the swelling has gone down and the scars have noticeably faded,
making this the time when the reduction in nostril visibility and the effects of lip-philtrum surgery
really begin to show.
In this patient’s case, the distance between the nose and the lips was clearly reduced,
making the facial impression sharper and more three-dimensional.
The nostrils also no longer rise when smiling.
The patient also said that there are almost no visible traces of surgery,
and that it settled in naturally after the operation,
and she regretted that she “loves it so much and wishes she had done it a little sooner.”^^

Front view / side view after 10 months
This is the 10-month mark.
By this stage, the surgical scar has faded a lot,
and this is the time when philtrum reduction surgery has fully settled in.
If you look at the patient’s photos,
even in the images, you can see that the central axis of the face has been harmoniously refined,
without any signs of surgery.^^
The philtrum also maintains a natural length that harmonizes well with the patient’s facial structure.
Philtrum reduction and nostril lowering are not simply surgeries that “make things shorter” and “lower things.”
They are advanced procedures that precisely fine-tune the central facial structure!!!!!
In cases like this patient’s, where the philtrum is long and the nostrils are tilted upward,
if surgery is done carelessly to shorten or lower them,
it can instead create an artificial expression when smiling,
or over time, disrupt facial proportions.
