Aegyo Under-Eye Surgery vs Lower Blepharoplasty for Aegyo: Which Under-Eye Procedure Is Right for Me?

Today, I’d like to professionally explain

the representative surgical methods for creating under-eye aegyo,

the recommendation criteria by case,

and the characteristics and detailed differences between the two approaches.

Under-eye aegyo surgery is broadly divided into two methods.

If you look at the image below,

The first is under-eye fat repositioning and aegyo fat grafting based on a conjunctival approach,

The second is lower blepharoplasty through a skin incision.

So why do the recommendation criteria differ?

Before and after under-eye fat repositioning + aegyo fat grafting

In general, if you are under 40 and have sufficient skin elasticity,

the preferred recommendation is under-eye fat repositioning + aegyo fat grafting,

which repositions the under-eye fat through the conjunctiva

and grafts an appropriate volume of fat to the aegyo area.

This method allows fine-tuning of the aegyo design to match each patient’s

needs,

and it is effective for creating a natural and lively aegyo look.

Before and after lower blepharoplasty

On the other hand, for those aged 40 and older, or when under-eye wrinkles and skin sagging are present,

lower blepharoplasty through a skin incision may be more suitable.

Lower blepharoplasty removes skin and the orbicularis oculi muscle, and after releasing the tear trough ligament,

it repositions the fat, allowing wrinkle improvement and restoration of skin elasticity at the same time.

In actual consultations, we comprehensively evaluate the skin condition, degree of wrinkles,

muscle development, whether there has been prior canthoplasty,

structural characteristics, and more,

then recommend the most appropriate surgical method in a customized way.

Now, let me briefly summarize what I’ve explained so far by looking at cases of patients who visited our clinic.

Before and after under-eye fat repositioning + aegyo fat grafting

The advantages of under-eye fat repositioning + aegyo fat grafting are:

• Conjunctival approach with no concern about visible scars

• Freedom to design the thickness, volume, and type of aegyo

• Suitable for people in their 20s to 30s with few wrinkles and firm skin

However, if there is a history of canthoplasty, please keep in mind that adhesions or

an uneven shape may occur as a result.

Before and after lower blepharoplasty

The advantages of lower blepharoplasty are:

• Skin excision and repositioning of the orbicularis oculi muscle and fat

• Suitable when moderate to severe wrinkles and skin sagging are present

• Helps improve wrinkles and restore elasticity

However, if too much skin is removed during surgery,

ectropion can occur, so a detailed surgical plan tailored to the patient

is necessary to prevent this in advance. And this is not about creating aegyo itself,

but about emphasizing the patient’s existing aegyo according to the development of their muscles!!

So especially in lower blepharoplasty, the structural foundation of one’s aegyo is used,

and depending on the orbicularis oculi muscle, the aegyo may appear more prominent,

or if the muscles are weak, as is often the case in men, the aegyo may appear relatively less noticeable.

For both methods, ideal results can only be achieved when the approach is detailed and strategic,

based on actual surgical experience and anatomical diagnosis, and tailored to each individual.
