Hello, I’m Lee Won, director of 21 Plastic Surgery.
It is said that facial lines are an important factor that greatly affects first impressions.
Among them, a double chin is a major cause of making someone look older
or giving a dull impression.
Many people think a double chin is mostly caused by fat,
so they try to address the cause by dieting as well as through procedures.
In cases where fat is the cause, there is a possibility of improvement,
but when a double chin is caused by various factors such as
salivary gland enlargement, platysma laxity, or skin sagging,
it may be difficult to resolve through dieting or procedures alone...
So how should we diagnose each patient in detail
and establish a surgical plan for the best result?
Today, I’ll introduce a case of a patient who visited us because of a double chin concern
and explain it in detail.^^

This patient was a woman in her 30s,
who visited 21 Plastic Surgery due to a particularly noticeable double chin.
When we examined her in the consultation room,
she had fat under the chin and in the jowl area,
loose platysma muscle, and salivary glands that were larger than average.
So the surgical plan was decided as follows.
Double chin and jowl liposuction, muscle tightening, salivary gland Botox

One week later, if you look at the patient’s front and side images,
you can see that the blurred facial line has been restored.
There was still some yellowish bruising and swelling,
but the heavy-looking double chin had noticeably reduced.

If you look at the front and side photos one month later,
the line between the face and neck has become clearly visible,
improving her appearance to look more natural and refined.

Some of you may be wondering here.
“Why is salivary gland Botox used in double chin surgery? What are the criteria for deciding to use it?”
I’ll explain the reason together with the photo below. ^^

The salivary glands are divided into the parotid gland, submandibular gland, and sublingual gland.
When the parotid gland is large, the side of the face can look puffy, and when the submandibular gland is developed,
a double chin and loss of jawline definition become more severe.
To reduce the salivary glands, salivary gland tightening or salivary gland Botox is performed.
The principle of Botox is that it blocks nerve transmission that makes muscles move,
thereby reducing muscle activity.
So when Botox is injected into the salivary glands, it also suppresses the nerve action involved in saliva secretion,
reducing salivary gland function.
Over time, the salivary glands also become smaller, and the facial line becomes smoother.
In conclusion, when improving a double chin and jawline,
a customized treatment plan that precisely identifies structural causes such as the salivary glands,
and combines salivary gland Botox when necessary, is the most important.
Only a plan based on each patient’s anatomical characteristics and actual cause
can reduce unnecessary procedural side effects
and ensure natural, high-quality results in the long term.
21 Plastic Surgery provides customized solutions through accurate cause diagnosis and the optimal surgical plan.
Our greatest goal at 21 Plastic Surgery is to help you regain your natural beauty.
