Real Case: Changes Before and After Philtrum Reduction + Philtrum Dimple Surgery, and Patient Review

Many people come in for consultations about philtrum reduction and ask,

“If I only have surgery, will my impression naturally improve?”

However, simply reducing the length of the philtrum does not always make the mouth area look natural and attractive. In particular, when the philtrum looks flat or wide, philtrum reduction alone may make the lips look rolled upward or the expression appear stiff. This case involves a woman in her late 20s,

who visited us with concerns that her face looked long when expressionless and that her thin lips gave a cold impression. Looking at the preoperative photos, the philtrum length was somewhat long,

and because the philtrum appeared flat and spread out, it looked like a shape without dimension. So we planned the surgery as follows.^^

Philtrum reduction (3 mm) + philtrum dimple surgery

The surgery was performed by naturally shortening the philtrum length, while designing an appropriate dimple line in the philtrum area to restore three-dimensional balance in the center of the face.

As you can see in the before-and-after photos, a natural sense of depth was created in the previously flat philtrum area,

and the connection between the lips, philtrum, and nose looks much smoother and more refined.^^ The lips also settled into a naturally rolled shape, without excessive curling,

and the overall expression became much softer and gave a younger-looking impression.
