Hello.
I’m Lee Won, the CEO head surgeon of 21 Plastic Surgery^^
A man’s double chin is often not simply a matter of “having gained weight.”
There are many cases where weight has barely changed,
or even when the person is consistently exercising
and managing their diet,
only the area under the chin looks unusually thick
and the jawline appears blurred.
Many men in these cases try to solve it with dieting or exercise,
but a man’s double chin is often not just a fat issue;
it is a complex structural problem involving
submental muscles, salivary glands,
sagging skin,
and even the structure of the jawline.
Before surgery, front/side
This case as well involved a male patient who came in worried that
although his weight was normal,
his double chin made his face look dull
and older.
As a result of the diagnosis, localized fat had collected under the chin,
and the muscles and salivary glands had also sagged downward,
leaving almost no visible boundary between the chin and neck.
Deep fat was also developed thickly.
With the goal of not just a simple procedure but “structural improvement,”
the following surgical plan was established.
Surgical plan
[Double chin liposuction + muscle tightening
- salivary gland tightening + skin excision]
1 week after surgery, front/side
One week after surgery,
there was still swelling,
but the fullness under the chin had noticeably decreased,
and the boundary between the chin and neck had begun to emerge gradually.
At this stage, residual swelling still means the line is not fully visible,
but compared with before surgery,
the structural change is clearly noticeable^^
1 month after surgery, front/side
One month after surgery,
as time passed, much of the swelling had subsided,
and the lower face looked more refined.
As the dull impression caused by the double chin improved,
a more masculine jawline began to reappear
from both the front and side views.

Six months after surgery,
the boundary between the chin and neck blended naturally,
and the mandibular line that had been blurred by the double chin
became clearly visible.
Without looking overly sharp or artificial,
the masculine jawline settled in naturally,
and from the front and side, especially in photos and videos,
you could confirm the change in which the lower face looked much more refined.
Before/after, front
If you haven’t gained weight but only a double chin remains,
the cause is more likely the structure under the chin rather than body weight.
There are limits to an approach that removes fat alone;
only by considering the muscles, salivary glands,
and jawline together can a more masculine impression be improved.
Before/after, 45 degrees
21 Plastic Surgery aims not merely to reduce a man’s double chin,
but to organize the structure of the lower face
and create a clearer, more trustworthy impression.
