Hello.
I’m Lee Won, the chief director of 21 Plastic Surgery.^^
When consulting about nose surgery,
I often get asked,
“If I change only my nose, will my overall facial impression change too?”
In fact, the nose is a structure located at the center of the face,
so changes to the nose are not just about the bridge or
the tip of the nose,
but directly affect the overall flow of attention and proportions of the face.
In particular, when the eyes and mouth are relatively distinct, but
the center of the nose is weak,
or when the facial center axis is off,
the lower face may appear heavier,
or the double chin may seem more emphasized.
Today, I’ll explain in detail through an actual case how
combining nose surgery and double chin improvement
changed the facial center and overall impression.^^

Front/side before surgery
The patient who visited us this time
was a woman in her late 20s.
She had previously undergone nose surgery,
but it had not improved properly,
and she came in with concerns that her double chin looked more prominent.
Based on an accurate diagnosis,
the nose’s support structure was weak and the sense of center was reduced,
which caused the eye line of the face to shift downward,
making the cheeks, chin, and double chin area
appear more prominent than they actually were.
So the surgical plan was decided as follows.
Surgical plan
Revision nose surgery + liposuction (double chin, cheeks) + muscle tightening + salivary gland Botox

Front/side at 1 week after surgery
One week after surgery,
there was still swelling around the nose and chin,
but from the front, the facial center became clearer,
and the overall impression began to look more organized.

One month after surgery,
as time passed, much of the swelling had gone down,
and as the nose line stabilized,
the three-dimensional quality of the face began to come through.
At the same time, as the cheek and jaw lines were refined,
the heavy lower-face impression caused by the double chin
was noticeably relieved.

Three months after surgery,
most of the swelling and residual swelling had subsided,
and the tissues were beginning to stabilize.
As the center of the nose became clearly defined,
the face’s dimensionality came back,
and the cheek, chin, and double chin lines were also refined,
changing the lower face into a much lighter
and more balanced impression.^^
The nose and jawline naturally harmonized,
confirming a clearer facial center from both the front and side.

Front before/after surgery
The biggest reason the impression changed in this case
was that we did not change only the nose,
but designed the facial center and lower face together.
The nose draws the eye upward,
and the double chin and jawline support that change stably,
making the overall impression much lighter and more refined.

21 Plastic Surgery
does not view the nose separately and contouring separately,
but places importance on designing while considering the overall flow and proportions of the face together.

If you are considering nose surgery,
rather than simply thinking about how much to raise the nose,
it is important to first look at what role the nose plays at the center of the face,
and whether it is in good balance with the lower face.
Through an accurate diagnosis that considers the harmony of the entire face,
we will guide you toward the most natural
and satisfying change.
If you have any questions,
please feel free to contact us for a consultation at any time.
Thank you.
