A Lip-Philtrum Reduction Surgery Case: Why Philtrum Dimple Surgery Was Performed Together

some people appear clear and well-balanced,

while others may look somewhat long

or have a center that seems to be pulled downward.

The factor that creates this difference is often

not the more noticeable areas like the eyes or nose,

but rather the facial midline and proportions.

Among these, the philtrum serves as a reference point

that divides the face into the upper, middle, and lower sections,

so its length and shape can easily change

the overall impression of the face.

Philtrum dimple surgery

creates a natural dimple line in the philtrum area

between the nose and upper lip,

helping add dimensionality to the center of the face.

If lip-philtrum reduction is about ‘adjusting length,’

The case introduced here involved a philtrum that was somewhat long,

and the space above the upper lip was flat,

so the surgical plan was set as follows.

Lip-philtrum reduction (5 mm) + philtrum dimple surgery

One week after surgery,

although swelling was still present,

the change in facial proportions was already visible

as the philtrum length became shorter.

A dimple line had begun to form in the center of the philtrum,

making the area above the upper lip stand out more clearly than before

and showing the direction toward a more defined center of the face.

Two weeks after surgery,

as the swelling gradually subsided,

the shape of the philtrum became more naturally visible.

The line formed by the philtrum dimple surgery

settled in without appearing excessive,

and the dimensionality between the upper lip and the nose

was expressed in a stable way.

In this way, when lip-philtrum reduction and philtrum dimple surgery

are performed together,

a much more refined result can be expected n than when only the length is reduced.

However, philtrum dimple surgery

is not a procedure that applies equally to everyone,

and it is important to perform it together only after

carefully analyzing the shape of the philtrum and facial proportions.
