
Before/after fat grafting
Hello.
I’m Lee Won, the director of 21 Plastic Surgery.
These days, fat grafting has become as common as double-eyelid surgery.
In consultations, some people even say,
“Since some of it will be absorbed anyway,
please graft a little extra.”

But fat grafting is not simply a procedure that involves putting in a lot.
In the end, this surgery is a battle with the engraftment rate.
Just because more is added does not mean it will all survive.
That is why PRP is sometimes mixed in,
and stem cells are sometimes used.
But there is one very important factor that many people overlook.
That is fat oil.

When fat is harvested,
it is not only pure fat cells that come out.
During the harvesting process, some fat cells are damaged,
and the triglycerides inside the cells are released and separated in the form of “fat oil.”
The more of this fat oil there is, the lower the fat engraftment rate becomes.
But the problem does not end there.
What happens if this oil is not absorbed at the graft site
and clumps together in one place?

At first, it can be felt like a small nodule,
and then it may gradually become harder.
In some cases, warmth, swelling, and pain may also be accompanied.
At that point, patients are often startled and ask,
“Could it be cancer?”
But in most cases,
it is neither cancer nor a tumor.
It is simply a clump of oil that was not absorbed.
If this oil remains,
it is not good for engraftment either
and can make the facial contour uneven.

That is why the most important step in fat grafting is
“refinement.”
21 Plastic Surgery does not stop at a simple single centrifugation.
After removing as much fat oil as possible through the first centrifugation,
a second short centrifugation is additionally performed.

The important point here is
that longer centrifugation is not necessarily better.
Excessive spinning can damage even normal fat cells,
which may actually lower the engraftment rate.
We set the optimal time and intensity so that
fat cells are preserved as much as possible
while only the fat oil can be selectively separated,
and then carry out the refinement process.
Fat refined in this way
has less unnecessary oil content,
minimizes lumpiness after grafting,
and allows for a more even engraftment rate.
Even so,
if there is already a formed fat oil nodule,
we can break it down without incision through LSSA ultrasound
and promote natural absorption.^^

Fat grafting
is not a matter of how much is put in,
but of how well it has been refined and managed.
If you feel a lump after fat grafting,
please do not try to press on it yourself or remove it yourself.
