I am Lee Won, the chief surgeon at 21 Plastic Surgery.
When consulting about lower blepharoplasty,
I often get asked,
“Can’t you just remove the protruding under-eye fat?”
But lower blepharoplasty is
not simply a surgery to reduce under-eye fat.
The key is to smooth the contour under the eyes by
removing protruding fat and filling in sunken areas.
And the structure that plays an important role in this process is
SOOF lifting.

Looking at the facial structure before surgery,
the under-eye fat was protruding forward,
and shadowing was formed in the tear trough area below it.
At the same time, the upper cheek volume appeared relatively drooped,
so the line extending from the under-eye area to the upper cheek
did not connect naturally.
In a structure like this,
simply removing the under-eye fat can
make the under-eye area look hollow instead,
or make the midface appear flat.
That is why, in lower blepharoplasty,
rather than unconditionally removing fat,
a combined approach that considers both redistribution and augmentation is needed.
So the surgical plan was set like this.

After surgery,
the protruding under-eye fat was smoothed out,
and you can see that the shadow in the tear trough area has been softened^^
Also, through SOOF lifting,
the upper cheek volume was reinforced,
so the line extending from the under-eye area to the upper cheek
became more naturally connected.
In this way, in lower blepharoplasty,
it is important not only to make the under-eye area flat,
but also to address the midface structure together!!

The key to lower blepharoplasty is
not simply removing protruding fat,
but redistributing the fat that is needed
to fill in sunken areas.
The structure that plays an important role here is
SOOF (Sub-Orbicularis Oculi Fat).
SOOF refers to the upper cheek fat layer
located below the redistributed under-eye fat.
Even after under-eye fat redistribution,
if the upper cheek volume is insufficient or there is sagging,
the SOOF is lifted upward
to reinforce both the under-eye area and the upper cheek at the same time.

To sum up, lower blepharoplasty is not simply
a surgery to remove fat.
To achieve a natural result,
you need to consider the under-eye fat, tear trough,
and upper cheek fat (SOOF) together.
