When the philtrum looks long,
it can lead to concerns that the overall impression seems flat.
Some people also feel that when they take photos,
the center of the face looks empty, or
that their lips do not look natural when they smile.
In many of these cases,
not only the philtrum length itself,
but also the shape of the philtrum area and
the sense of dimension are affecting the result together.
That is why patients who visit us while considering philtrum reduction often ask during consultation,
“Would it be enough to just shorten the length?”
Depending on the face,
there are also cases where adjusting the length alone is not enough.
Today, I’ll introduce a case of philtrum reduction + philtrum dimpling surgery,
and explain philtrum dimpling surgery in detail as well. ^^

Before surgery,
after the patient visited and we made a detailed diagnosis,
the philtrum was relatively long, and the center of the philtrum was flat,
so the overall impression made the center of the face look somewhat empty.
From the front, the face looked longer,
and from the side, the connection between the lips and the nose
felt rather monotonous...
In this case, if only the philtrum length were reduced,
the proportions might improve,
but because the lack of dimensionality could limit the visible change,
we planned to combine philtrum reduction with philtrum dimpling surgery.

One week after surgery,
although swelling was still present,
we could confirm that the shortened philtrum had helped bring the center of the face upward into a more organized flow.
In particular, the dimpling line formed at the center of the philtrum made the face look less flat from the front,
and the connection between the lips and the nose appeared more natural.^^

One month after surgery,
the swelling had largely subsided,
and the effects of philtrum reduction and philtrum dimpling surgery
were more clearly visible.
The philtrum had become shorter, but it did not look compressed;
rather, shadows and contours formed in the center of the face,
and the overall impression began to look much more three-dimensional.
When smiling, the lips curved more softly,
and in photos as well, the face appeared sharper.
This change was not simply the result of a “shortened philtrum,”
but of a design that considered both shape and structure together.

At 21 Plastic Surgery,
we do not view philtrum reduction as simply a surgery to shorten length.
We consider the overall facial proportions, the length of the midface,
the thickness of the lips, and the movement of the facial muscles when expressing emotions,
and design both the philtrum shape and its dimensionality together.
In particular, philtrum dimpling surgery is not approached on its own;
after analyzing the philtrum’s length, width, and whether it is flat,
it is combined with philtrum reduction to complete the structure of the center of the face.

This kind of approach is necessary
so that the result does not feel awkward even as time passes after surgery,
and so that a natural change in impression can be expected.
