Dark Circles and Under-Eye Fullness—Addressed Together at 21 Plastic Surgery!

As the days get brighter and outdoor activities increase,

the shadows created by under-eye fat

become more noticeable under the clear sunlight,

making the entire eye area look tired more easily..

But there is a way to address

these concerns all at once.

Today, I’d like to talk about it^^

Why do the under-eyes look so tired?

There are usually three causes that work together

to make the area under the eyes look tired and aged.

The first is bulging under-eye fat.

As we age, the fat surrounding the eyeball

moves forward,

causing the under-eye area to bulge and creating darker shadows beneath it.

The second is a deeply indented tear trough.

The more the fat bulges, the deeper the boundary beneath it

appears, making dark circles more noticeable

and giving the entire eye area a heavy appearance.

The third is the absence of under-eye fullness.

Without the softly rounded fullness just beneath the eyes,

the area can look flat

and it becomes difficult to create a lively impression.

We design three procedures as one.

Under-Eye Fat Repositioning

We approach through the inside of the conjunctiva,

spread the protruding under-eye fat as thinly and widely as possible,

and evenly fill the hollow tear trough.

Rather than simply moving the fat,

we spread it out widely before filling the area,

so the hollowed-out region is filled much more naturally.

Under-Eye Fat Grafting

For areas that cannot be sufficiently filled through repositioning alone,

we supplement the treatment by grafting the patient’s own fat.

It is particularly effective for people with deep tear troughs,

and its advantage is that the natural volume

can be maintained for a long time.

Under-Eye Fullness Fat Grafting

We finish by grafting fat along the under-eye fullness line.

A softly rounded volume develops beneath the eyes,

completing a gentler and more endearing appearance.

When these three procedures are performed as one surgery,

the overall impression of the eye area changes all at once.

Under-Eye Fat Repositioning + Fat Grafting (Under-Eye Fullness, Under-Eye Area)

Let’s look at an actual patient case.

This patient had protruding under-eye fat

and deeply indented tear troughs,

and had often been told that they always looked tired.

Looking at the frontal photo before surgery,

the under-eye area protrudes forward,

and the boundary beneath it

appears distinctly dark.

The under-eye line was sunken,

revealing a structure that looked flat.

There was also almost no under-eye fullness, making the eyes look less defined,

and the overall impression was cold.

For this patient, along with under-eye fat repositioning,

fat grafting to the under-eye fullness and under-eye areas

was performed as a combined treatment.

The recovery process—this is how it proceeds!

Because the incision is made inside the conjunctiva,

there is almost no visible scarring.

There may be swelling and bruising during the early postoperative period,

but it is possible to return to daily activities within 1–2 weeks,

and the final result can be assessed between 1–3 months.

When fat grafting is performed as well,

the grafted fat needs time to settle into place!

The volume may initially look somewhat excessive,

but it will naturally stabilize over time.

Use cold compresses to manage early swelling,

and avoid rubbing or strongly irritating the eyes.

Keeping your head elevated while sleeping

can help with recovery.
