Hello.
I’m Dr. Lee Won, director of 21성형외과.
Facial contouring surgery is a procedure that improves the size and shape of the facial bones, but bone structure is not the only factor that determines the surgical outcome.
Even when the same cheekbone reduction or square-jaw surgery is performed, the degree of visible change can differ from patient to patient.
One of the important factors creating this difference is the thickness of the soft tissue and fat layer covering the face.
Today, let’s take a look at how the amount of facial fat affects the results of facial contouring surgery.^^

Facial contouring surgery reshapes the cheekbones, square jaw, chin, and other facial bones to refine the facial line.
However, the face we actually see is not the bone itself, but the subcutaneous fat, soft tissue, and skin covering it.
Therefore, even if the shape of the bone changes, the visible external result may vary depending on the amount of facial fat and the thickness of the skin.
In particular, when there is a large amount of facial fat and the skin is thick, some of the changes to the bone may be obscured as they are transmitted to the surface of the skin.
Conversely, when the facial fat is thin and there is little subcutaneous fat, the changed contour is revealed more directly, so the facial line often appears more defined.

The purpose of facial contouring surgery is important here.
Many people think of facial contouring surgery as a procedure to make the bones smaller, but simply reducing the bones is not the actual goal.
The reason we often feel that someone has a “beautiful facial line” or that their “face shape looks more refined” is not simply because of the size of their bones, but because the contour appears naturally and smoothly.
Ultimately, the goal of facial contouring surgery is to create a more balanced face shape by ensuring that the changed bone structure is expressed well externally.
[Procedure Plan]
Three-part facial contouring (cheekbones, square jaw, chin)

Let’s look at an actual patient case.
This patient had prominent lateral cheekbones and a square jaw, which made the lower face appear heavy. She wanted a softer and more sophisticated face shape.
In particular, this patient had relatively little facial fat and comparatively thin skin.
When there is less facial fat, the bone contours are structured in a way that allows them to show clearly on the outside. As a result, the facial line changed by surgery tends to be expressed more distinctly.

After surgery, the prominent cheekbones, square jaw, and chin were naturally refined, reducing the horizontal width of the face.
The previously uneven contour became smoothly connected, and the overall impression also changed to appear more feminine and sophisticated.
Thanks to the structural characteristic of having less facial fat, the bone contours changed by surgery were clearly visible externally. This can be considered a representative case in which the effects of facial contouring surgery are easy to observe.^^
Then what can be done when there is more facial fat?
When there is a large amount of facial fat, facial liposuction may be considered in combination with facial contouring surgery.
By refining the size and shape of the bones through facial contouring surgery while appropriately reducing facial fat at the same time, the changed contour may become even more clearly visible.
If you have any additional questions, consultations are available anytime~
Thank you.
