Many people first try thread lifting or
laser treatments.
At first, they feel that the treatments are effective, but with repeated procedures,
they begin to feel that the results are less significant than expected.
Today, I’ll explain why procedures alone have limitations
and how a facelift
can go beyond them.

Aging begins in the underlying structures, not the skin
Many people think of aging
only as a loss of skin elasticity,
but in fact, it begins first in deeper layers.
Under the facial skin
there is a structure called the
*SMAS (superficial musculoaponeurotic system), as well as
retaining ligaments that secure the full thickness of the skin to the bone.
As we age, these retaining ligaments stretch
and weaken, causing the SMAS and skin
to begin settling downward under gravity.
Sagging cheeks, deeper nasolabial folds,
and a weakened jawline
all originate from this process.
Because thread lifting and laser treatments act only on the skin layer,
they cannot reach these deeper structures,
which is why the tissues can quickly return to their original position even after being lifted.

I believe the most important step in a facelift
is sufficiently releasing the retaining ligaments.
Before lifting the skin and SMAS,
these retaining ligaments must first be adequately released
so that excessive tension is not concentrated on the skin
and the tissues can be naturally repositioned
in the desired direction.
If the retaining ligaments are not released sufficiently,
even strong traction may quickly result in the tissues returning to their original position,
or the effects may not last long.
Conversely, after thoroughly releasing the retaining ligaments,
if the SMAS and skin are lifted together,
the underlying cause of the sagging itself is addressed,
which can provide a more definite effect
that lasts longer.
I also pay close attention to designing the incision sites.
I design the incisions along the inside of the hairline and the contours of the ears
so that the surgical marks are concealed
as much as possible.
This is always an important principle for me.
After recovery, the incisions are naturally concealed
by the hair and ear lines,
making it difficult to tell
that the person has undergone surgery at all.

Full facelift and neck lift + liposuction (front cheeks, double chin) +
muscle and salivary gland tightening + skin excision

This patient was a woman in her 40s
who had undergone various lifting procedures over several years,
but visited the clinic because her sagging cheeks, nasolabial folds,
and weakened jawline had not improved significantly.
In the before-surgery photos,
you can see that the cheek fat had settled downward,
creating deep nasolabial folds.
From the side, the boundary between the jawline and neck was indistinct.
Overall, she had an appearance
that made her look about 10 years older.
After subcutaneous dissection, the retaining ligaments were sufficiently released,
and the surgery was performed by lifting the SMAS and skin together.

Two months after surgery,
the sagging cheek fat had been lifted back to its original position,
greatly softening the nasolabial folds.
The previously weakened jawline seen from the side
became distinct and clearly defined.
A facelift is not simply
surgery that pulls the skin.
It is surgery that fundamentally lifts
the facial structures that have settled downward with age.
