One of the areas where changes first
begin to appear with age is the area around the eyes.
In particular, middle-aged men often visit a clinic after hearing comments such as
“you look tired,” “you look angry,” or
“you look older than your actual age.”
However, many people think these changes are simply due to skin aging.
In reality, they usually occur as the skin beneath the eyes, fat, muscles, and supporting tissues all change together.
Today, I will explain lower blepharoplasty through the case of a middle-aged male patient.

Lower blepharoplasty is not a procedure that applies to all types of under-eye aging.
For younger patients with only protruding under-eye fat, sufficient improvement may be achieved with under-eye fat repositioning alone.
On the other hand, after middle age, reduced skin elasticity often causes the skin beneath the eyes to sag, while fat protrusion and wrinkles appear together.

When skin sagging and fat protrusion progress at the same time, it can be difficult to achieve satisfactory results with fat correction alone, so considering lower blepharoplasty may be helpful.

In lower blepharoplasty, it is important not only to remove sagging skin but also to refine the under-eye area as naturally as possible.
The skin beneath the eyes is very thin and is an area with frequent facial movement. If too much skin is removed or the sides are not balanced, an unnatural appearance may remain after surgery, or ectropion may occur.
In addition, each patient has a different degree of skin sagging, fat distribution, and condition of the tissues supporting the eyelids. Therefore, planning tailored to the individual’s anatomical structure is necessary rather than using a uniform surgical method.
Only by fully considering these factors can a soft and natural eye area be maintained after surgery.

Lower blepharoplasty + SOOF + forehead lift

This patient was a middle-aged man with sagging skin beneath the eyes accompanied by fat protrusion.
From the front, the area beneath the eyes appeared heavy and droopy. At a 45-degree angle, the skin folded, creating a tired and fatigued appearance that made him look older than his actual age.
In addition, the under-eye fat created uneven contours, making the entire face appear heavier.
Because sufficient improvement would have been difficult with under-eye fat repositioning alone, we planned lower blepharoplasty to appropriately refine the protruding fat while also correcting the sagging skin.

For middle-aged men, improving changes around the eyes alone often makes the overall appearance look much brighter and clearer.
At our clinic, we thoroughly analyze each patient’s individual pattern of under-eye aging and plan surgery while considering the balance of the skin, fat, and muscles.
In male patients especially, we focus on naturally improving only the tired-looking aspects while preserving the existing appearance, so that the eyes do not become excessively taut or overly feminine.
The goal of surgery is not to create youthful-looking eyes, but to restore the patient’s original, natural appearance.
