Introducing the Twin Block Appliance Used Before PreOrtho for Children’s Recessed Chin Correction.
Hello,
I’m Lee Juri, the head dentist at Seoul Bardi Dental Clinic.
In children’s orthodontic treatment during growth, the PreOrtho appliance
has become really well known these days, right?
But before PreOrtho appeared,
there was an appliance that was widely used in growth-phase orthodontic treatment.
That is the Twin Block.
Today, I’ll introduce the Twin Block appliance, which is still widely used.

What is a recessed chin?

Before introducing the Twin Block appliance,
let me briefly explain
what a recessed chin is.
When the lower jaw is positioned slightly backward,
it is described as a recessed chin, making the chin look absent or short.

In general, the tip of the nose, upper lip, and lower jaw should be close to a harmonious straight line.
But because the lower jaw is set back more than normal,
it makes the chin look short from the side,
and the mouth may appear protruded or the jawline may look less defined.
It does not literally mean there is no chin ^^;;

How Twin Block looks when worn
Twin Block is an appliance with separate plastic blocks attached to the upper and lower teeth.
It is worn separately on the upper and lower teeth
and is removable.
When treating a child with a recessed chin, the lower jaw is guided forward as the blocks interlock with each other.
It cannot be used by adults
and is used only for children who are still growing.
For effectiveness, it needs to be worn for more than 20 hours a day,
which is most of the day.
Recently, PreOrtho appliances are also used
to treat recessed chin,
but
these two appliances have some differences.
If PreOrtho uses muscle function,
Twin Block controls bone growth.
Because Twin Block fundamentally promotes jaw growth,
it may be more effective when jaw growth is needed, such as in a recessed chin.
Since it is a custom-made appliance made by taking an impression,
its biggest advantage is that the degree to which the lower jaw is advanced can be adjusted.
As for the downside...
It must be worn for a long time.
Aesthetics are
just my personal opinion, but...
I think both appliances are similar.

PreOrtho is used a lot these days,
but as a ready-made product, it does have limitations.
It is difficult to adjust how much the lower jaw is advanced.

Ready-made PreOrtho
Soft, flexible material
Short wearing time
These two advantages
make it easier for children to adapt.
Rather than saying one appliance is good or bad,
it is determined by the child’s tendencies
and the current condition of the teeth and jaw.
The core principle of adjusting jaw position has existed for a long time.
Twin Block was a representative appliance for this,
and it is still a valid treatment option depending on the situation.
Even if the appliance changes,
the principle of helping a child’s jaw growth does not change.

Additionally, both Twin Block and PreOrtho
cannot align the teeth,
so
additional orthodontic treatment may be needed toward the end of the growth period.
If you keep that in mind,
I think it will be very helpful.
