Hello.
I’m Lee Juri, the chief director of Seoul Bardi Dental Clinic.
Today’s post is not relevant to patients who are currently undergoing clear aligner treatment.
“Doctor, this bracket came off again.”
Have you ever felt inconvenienced because the bracket attached to your tooth kept coming off repeatedly during wire braces treatment?
And especially when one particular tooth keeps coming off much more often than the others?
In many cases, brackets attached to prosthetic restorations such as crowns can come off repeatedly.
In fact, this is not the patient’s fault, nor is it necessarily the doctor’s mistake. It is a natural occurrence that can happen commonly during orthodontic treatment.
Today, I’ll explain why brackets attached to prosthetic restorations tend to come off so easily and how to prevent it.
Please read carefully for just 3 minutes.

Why brackets attached to prosthetic restorations tend to come off so easily

In orthodontic treatment, the small metal pieces attached to the surface of the teeth are called brackets.
During the bonding process, if moisture (water) gets in, or if you eat hard foods, they may sometimes come off.
Also, when the bite is tight, the applied force can cause them to come off repeatedly.
Among these, when the tooth is not natural but a prosthetic restoration, the appliance is more likely to come off easily.

The cause of bracket debonding on prosthetic restorations
The typical cause is the limitation of bonding strength.
When a bracket is bonded to the enamel surface of a natural tooth, the adhesion is strongest.
Prosthetic restorations such as crowns and veneers may be made of different materials, including ceramic and resin,
the common point is that their surfaces are smooth.
Even if adhesive is used to bond them,
there is no choice but for the physical bond to be weaker than the enamel of a natural tooth.

That is why when a prosthetic restoration chips, many people ask whether it can simply be repaired by adding material.
However, simple patch repair is almost impossible.
(Most of the time, it ends up being remade.)
To solve this problem,
adhesives made specifically for prosthetic restorations have also been developed, but...
because of the surface characteristics of prosthetic restorations,
debonding can still happen repeatedly to some extent.
- Do you have to keep putting up with the inconvenience? What is the solution?
Many patients visit the clinic every time a bracket comes off, but the inconvenience is real.

At the start of treatment, we show you the X-ray, explain the reason,
and even if we explain that it may come off often,
for busy office workers, coming in again every time it happens can be difficult.
Even if the appliance comes off, we can try re-bonding it using the bonding method two or three times.
We also pay even more attention to moisture control during the procedure.
In many cases, the bracket stays on until the end of treatment and we are able to finish successfully.

Metal band
However, if bracket debonding keeps recurring on a specific tooth,
we can try a "metal band."
In other words, we completely encircle the tooth with a device called a band.
Today, I explained the causes of orthodontic bracket debonding and how to address it.
Orthodontic treatment is not something performed only for teenagers,
so cases in which treatment begins after implant treatment or with several prosthetic restorations already present
are increasing gradually.
What is important is that if a bracket comes off, you should come in right away.
If it is left unattended, the force will not be properly applied to the tooth,
which delays tooth movement.
If a bracket attached to a prosthetic restoration keeps coming off,
it is a common situation during orthodontic treatment, so there is no need to be too alarmed.
There are various alternatives,
and there are enough ways to solve the problem.
For orthodontic force to be properly transmitted,
the appliance must stay firmly attached.
This has been Chief Director Lee Juri.
Thank you.
