What should you do when pus comes out of an implant and you feel pain?
Hello,
I’m Lee Ju-ri, the chief director of Seoul Bardi Dental Clinic.
There are quite a few patients who come in saying that the implant area they had placed hurts.
Usually, this is caused by the gums becoming inflamed,
swelling, and pain.
This happens because of the structural differences between natural teeth and implants.
Today, while explaining this difference,
I’ll introduce how to deal with pus coming out of the implant area.

- Inflammation caused by the difference between implants and natural teeth!
This was a patient who had received an implant at another dental clinic several years ago.
At first, they used it well without any problems,
but over the past few days, the gums had swollen
and they said they could taste pus.

When I examined the inside of the mouth,
I saw swollen gums around the area and bleeding.
Why does this happen?
Is it because they didn’t brush properly?
It’s because of the biggest difference between natural teeth and implants.
Natural teeth have a tissue called the periodontal ligament
between the root and the gums.
This acts like a cushion.

This periodontal ligament acts as a protective barrier,
preventing bacteria from burrowing into the root
and also absorbing chewing pressure.

But implants do not have a periodontal ligament.

Because the bone and implant are in direct contact,
if even a small gap forms,
food particles and bacteria can enter directly.
Compared with natural teeth,
the space between the gums is also not as tightly sealed.
Since brushing does not perfectly clean the gaps around the implant and gums,
inflammation progresses quickly,
and in severe cases, pus can also form.
- How can implant inflammation be treated?
If you visit in the early stage,
even simple cleaning and a prescription can help.

This patient had been a long time out from implant surgery,
and the bone around the implant had melted away significantly,
to the point where the screw portion was exposed.
We need to remove the existing implant
and place a new implant.
If they had come in earlier,
if they had at least come in for regular checkups,
we would not have had to remove it..
I was sorry about that.
Because implants do not have a periodontal ligament,
they require much more careful care than natural teeth.
Even cars need regular oil changes
and inspections.
Implants also need checkups,
even if nothing feels uncomfortable.

Clean around the implant with interdental brushes,
and if you feel even a little discomfort,
you need to visit the clinic right away.
If you come in early, treatment can be simple,
but if pus is left untreated, the bone can also dissolve and you may eventually need to remove the implant.
Because implants do not have that protective barrier, even small problems spread quickly.
There is no guarantee that if it’s fine today, it will still be fine tomorrow.

Even if there is no special problem with the implant area,
if it has been a long time since your last dental visit,
I recommend coming in at least once,
and I’ll end this post here.
Thank you.
