Today is the story of a patient who visited because they could not chew.

My lower right tooth hurts terribly even when it barely touches something

That was the patient’s first comment when they visited today.

It wasn’t that it hurt badly when brushing or chewing hard food,

but rather that even a slight touch between the teeth

caused severe pain.

After taking an X-ray,

there was inflammation at the end of the tooth root~

And the prosthetic shape is unusual, right~?

It is a cantilever bridge.

It refers to a bridge that spans a missing space by being supported on only one side tooth.

Common bridge

Normally, a bridge uses the teeth on both sides of the missing tooth as supports

and connects them like a bridge,

but

a cantilever bridge uses only one side tooth as the support,

so the other side is left floating in the air.

The patient had two premolars

supporting a molar~

Because it does not require shaving down the teeth on both sides,

the amount of tooth reduction is smaller...

However...

all of the chewing force is concentrated on one tooth.

Since molars are subjected to such strong forces,

the tooth root and jawbone can become overloaded and deteriorate quickly.

Over time, the tooth may become loose,

and inflammation can develop at the root tip, often eventually requiring extraction.

Because there were two fewer teeth than on the opposite side,

the overload happened quickly~

2. Learning about the treatment process

At first, we removed the crown

and checked the internal condition.

There was decay inside,

and there was also inflammation at the root tip,

but through root canal treatment,

we decided to try to save it as much as possible.

And the prosthetic that had been replacing the molar

was planned to be treated with an implant.

When inflammation occurs at the tip of a tooth root,

an abscess forms around it.

At this time, when chewing force is applied,

it is like pressing on a wound,

even a small amount of pressure can cause severe pain.

When the inflammation is drained through root canal treatment,

the pain is greatly reduced.

We restored the molar function with an implant,

and performed root canal treatment on the tooth with inflammation,

repairing the remaining teeth so they would not collapse further.

Especially when relying on a prosthetic such as a bridge,

it is common for the supporting tooth to suffer root damage due to overload.

Although the patient came in with pain that occurred even from a slight touch,

we restored each tooth so it could perform its own function,

and now the only thing left is to use them strongly for a long time.

Today, when a tooth hurts even when it only touches something,

we looked at why it hurts

and how we resolved it.

Like today’s patient,

tooth pain often means

there is likely a problem inside.

Trying to solve it with medicine,

or just enduring it and thinking, “It’ll pass~,”

may be the beginning of a vicious cycle.
