Hello!
I am the director of Seoul Bardi Dental Clinic.
Today, I would like to share a somewhat special case.
No permanent teeth?
Missing permanent teeth
Example case

Usually, when a baby tooth gets loose,
it falls out and an adult tooth (permanent tooth) erupts,
but there are cases where the permanent tooth itself is missing.
This is called a missing tooth.
Many people have this condition
without realizing it.
These days,
people often get X-rays from a young age
and go for regular checkups,
so many already know they are in this situation..
If you simply pull out a baby tooth because it is loose..
sometimes it ends up staying empty.
That is why we take X-rays
and check whether there is a permanent tooth bud.
This patient’s mouth was the same.
The baby tooth had remained in place until adulthood,
and after trying to preserve it as long as possible,
it had now become so loose that treatment was needed.
Example situation
No permanent tooth

Detailed examination results
Baby tooth remains in the front tooth area
Congenital absence of a permanent tooth in that area confirmed
Short baby tooth root with ongoing resorption
Severe mobility
In cases like this,
many people already know
that the permanent tooth is missing.
Congenital missing tooth characteristics
No permanent tooth
Occurs in about 5–10% of the population
Mostly seen in the lateral incisors and premolars
Baby teeth have shorter and weaker roots than permanent teeth,
so they should be retained as long as possible, then removed, and the empty space treated.

If left untreated after falling out,
the tooth alignment can collapse like dominoes,
so it is important to receive treatment without missing the right timing.
When a congenital missing tooth is diagnosed,
it can be surprising,
and you may wonder, why only me..
but if you preserve and manage the baby tooth for as long as possible,
then when its limit is reached, you can consider the timing for treatment.
Just as other teeth receive appropriate treatment when problems arise,
missing teeth can also be used without issue if they are properly treated.
Choosing a treatment method
No permanent tooth
If one tooth is missing, there are broadly two treatment options
- Bridge
: A disadvantage is that the healthy natural teeth on both sides must be shaved down
If retreatment is needed in the future, all three teeth must be addressed
Because the front tooth area is important for aesthetics,
this approach requires even more caution
- Implant
: Independent treatment is possible
Most stable in the long term
After sufficient consultation with the patient,
the following treatment plan was established
Extraction of the baby tooth + immediate implant placement
Bone grafting performed
For the front teeth, the gum line was shaped with aesthetics in mind

The front teeth are the most visible area when smiling,
so aesthetics and natural appearance
were given top priority.
24.2.23
While extracting the baby tooth,
an implant was placed
Since there was a gap in the middle,
a temporary tooth was made so it could be used in the meantime
After checking the implant condition 6 months later,
an impression was taken
One week later,
the tooth was inserted.
It was temporarily attached,
and we checked whether there were any uncomfortable areas
or anything that needed improvement.
Because the color seen inside the clinic
and the feel seen outdoors can be different,
we had the patient try it sufficiently
before proceeding with final bonding.

24.11.5
After checking the condition,
and hearing that it was fine,
the treatment was completed, and another check-up was scheduled for one week later
Timing of treatment for patients with missing teeth
No permanent tooth
When the baby tooth is healthy
-> Monitor the condition through regular checkups
When the baby tooth starts to become loose
-> Start establishing a treatment plan
When the baby tooth has severe mobility or pain
-> Proceed with immediate treatment
Treatment method selection
-> If a single tooth is missing
implant treatment should be considered first
Preserving healthy teeth is the top priority

Congenital missing teeth
are a problem that more people experience than you might think.
So please do not worry too much,
and if you choose the right treatment method
at the right time,
you can maintain healthy teeth.
From baby tooth preservation to implant treatment,
we will help you receive treatment with peace of mind,
step by step.
Thank you
