When patients visit the dental clinic,
they usually come for two main reasons.

Gum (periodontal) disease
According to statistics, more than about 70% of adult patients
have periodontal disease,
and especially after the age of 40,
the biggest cause of tooth loss is periodontitis.
Cavities are visible and painful,
so people tend to visit relatively quickly,
but gum disease has only mild symptoms at first
and progresses slowly,
so there are many cases where people end up visiting the hospital
only after it has become a serious condition.
The case I’m introducing today
is exactly one of those cases.
This is the story of a patient in their 60s
who came in with pain caused by molars shaking
because of chronic periodontitis.
This case was also one that reminded us once again
of the importance of gum care.
Condition inside the mouth at the time of the visit

The examination results were as follows.
- Overall progression of chronic periodontitis
- Severe pain in the lower right molar
- Overall tooth mobility confirmed
- Poor bone condition due to alveolar bone resorption caused by periodontitis
- The upper teeth were in an overdenture state (at another dental clinic)
- Extraction was unavoidable
- The patient said the upper teeth were not causing much discomfort
Why periodontitis is frightening

Periodontitis is called a silent disease.
It starts with mild symptoms such as swollen gums
and bleeding in the early stages,
but if left untreated, it eventually
melts away even the bone that supports the teeth
and ultimately leads to tooth loss.
In this patient’s case,
the teeth were all loose due to periodontitis,
and extraction was the only option.
The upper jaw was already using dentures,
and if the lower jaw were treated with implants,
it would be difficult for the force to be distributed evenly.
The denture area is relatively weaker,
while implants are somewhat stronger.
For this patient’s condition,
a denture or implants were the best options,
but I do wonder how things might have been
if care had been taken in advance.
As I see patients like this every day..
I can’t help but emphasize gum care even more.

Molar extraction
Because there were many out-of-town schedules,
the patient wanted to proceed a little quickly.
For making the temporary denture,
a mold was taken
and on 25.5.1
full lower-jaw implant treatment was performed.
Bone grafting was also done together.

Since there was still swelling,
there would continue to be discomfort in some areas.
Due to out-of-town schedules,
a follow-up visit was planned after 2 months.

Even though the interval between visits was long
because of out-of-town schedules,
the patient said that thanks to the temporary denture,
they were able to spend the recovery period
without major problems in daily life.

Gum care is important even if you only have implants.
Myeong-dong Implant
Now there are no natural teeth
left in the mouth.
Natural teeth have regenerative ability
and can recover on their own,
but implants do not.
That is why you need to brush even more carefully,
and for areas that the toothbrush cannot reach,
use dental floss or interdental brushes
for thorough cleaning.
And the most important thing is,
even if there is no discomfort,
visit the dental clinic at least twice a year
and get checked to make sure there are no issues with the teeth.
