There are two main causes of the tooth neck area becoming notched.
One is improper brushing habits, such as brushing too forcefully from side to side,
and the other is
excessive biting force (occlusal force) applied to the teeth.
If the filling keeps falling off, you should pay attention to the latter:
"occlusal force."
Every time the upper and lower teeth come together,
the teeth bend very slightly to the side.
But if the chewing force is too strong
or only certain teeth are biting hard,
then the enamel in the weakest area, the tooth neck (cervical area),
starts to break away.

The professional term for this is
"abfraction."
This goes beyond simple surface wear;
it is a structural problem caused by the tooth bending under occlusal force.
No matter how firmly resin is bonded to a tooth
that keeps being hit like it’s being hammered,
every time excessive force is applied,
the resin has no choice but to pop off.

A patient who visited our clinic recently
was deeply concerned because the resin kept falling off.
There was cervical abrasion overall,
but
in particular, the two upper left teeth (#22 and #23)
were especially deeply notched.

Because the notched area was quite large,
the nerve had already been exposed.
Why were these two teeth
so severely notched?
The reason became clear as soon as we looked at the lower teeth.

The patient ate much more on the left side than the right,
and the lower left teeth were severely worn down,
so the chewing force on the left side was overwhelmingly stronger.
In a structure where occlusal force is concentrated on one side like this,
it may be natural that even a carefully done filling
falls off again after just a few days.

Because the notched area was already too large
and the nerve was exposed,
simply "filling it" was no longer meaningful.

So we planned crown treatment after root canal treatment.
However, there was one major variable here:
the second front tooth (#22).
The neighboring canine (#23) had a deep root and strong bone support,
so after root canal treatment, placing a crown on it
would allow it to be used for a sufficiently long time,
but
the small front tooth next to it (#22) was a bit different.

Compared with the canine, the bone around the root had resorbed significantly,
and mobility was already observed.
We could perform root canal treatment
and force a crown onto it,
but
how long would this tooth really be able to withstand strong occlusal force?
Rather than spending unnecessary money and time
on a tooth that would likely need to be extracted soon,
we decided that an implant would be better for the long-term prognosis.

We explained the current condition honestly to the patient,
and in the end,
we decided to save the canine with root canal treatment and a zirconia crown,
and proceed with an implant for the mobile small front tooth.

Since the implant and crown would be placed side by side,
the harmony with the surrounding teeth was especially important.

Using the patient’s occlusal data,
which we analyzed in detail,
I and the head technician communicate closely
by sharing information in real time.
To disperse the strong force concentrated on the left side,

we not only designed the shape and angle of the prosthesis
with great precision,
but also reproduced the color and texture of the surrounding teeth as closely as possible.
Using high-strength zirconia that is suitable for patients with strong biting force,
we made it so they could eat comfortably without worrying about fracture.

Just because a tooth is notched does not mean
that filling it with resin is always the best answer.

"Why did this area become so notched?"
