or anxious that the tooth might fall out right away,
so they often want to use a loose tooth by somehow ‘just fixing it’.
You may feel relieved once the tooth is stabilized for the moment,
but in fact, covering up the outside without knowing the cause
can be a very dangerous choice.
Ignoring the warning sign that the jawbone is collapsing
and hiding it with a temporary fix
is like tying a falling tree loosely with a rope
when the soil supporting the roots has already washed away.
It may look like it is standing fine on the surface,
but in the end, there is no way to prevent the tree itself from being uprooted.
The patient I’m introducing today also came in
with a similar mindset at first.
But after the examination,
unfortunately the jawbone around both molars had
already dissolved to the point where it could no longer support the teeth,
so full reconstructive treatment was unavoidable.
For a fundamental recovery, not a temporary fix,
what concerns and processes were involved in treatment?

A loose tooth means that the jawbone holding the tooth,
that is, the alveolar bone,
is being lost.
If we compare it to a building,
it means the ground beneath it is shaking.

What happens if you tightly bind the teeth with resin
without resolving the cause?

If you artificially seal the gaps,
small food particles will continue to get between the teeth.
The problem is that there is no way for those remnants
to get back out.
They rot inside
and cause severe inflammation.

In the end, this inflammation
without the patient even realizing it,
destroys the remaining jawbone at a frightening speed.
When is splinting needed?

Loose teeth do not always have to be extracted.
The choice can vary depending on the situation.
Also called temporary stabilization,
this is a method of binding the tooth with adjacent teeth
like shoulder-to-shoulder support to reduce mobility and pain.

It is very useful for dental trauma or emergency treatment.
However, the basic condition that there must be no inflammation in the tooth is very important.
Above all, postponing fundamental treatment such as root canal treatment or extraction
while keeping only the splinting in place for a long time
is by no means a good method.
If the bone has already dissolved all the way to the root tip
and there is no support left for the tooth, with severe inflammation,
then replacing it with an implant before the surrounding bone deteriorates further
is much more advantageous in the long term.

When the patient first visited,
the condition was much more serious than expected.
It had already gone beyond the stage where it could simply be bound and used.

Most of the upper molars on both sides,
as well as the lower right molar,
were barely holding onto the tooth roots
because chronic inflammation had melted away the jawbone.

In particular, the upper molar area had bone thickness toward the maxillary sinus (the empty space beside the nose)
that had become as thin as paper,
so even the basic groundwork for placing implants was difficult.

After carefully analyzing the bone condition through CT imaging,
we established a treatment plan to rebuild the deficient bone first.

Bone grafting through bilateral sinus lift procedures
The biggest task in this treatment was
securing space for implant placement!
We carefully lifted the very thin sinus membrane,
like an eggshell,
and performed a sinus lift procedure by firmly filling the empty space with bone graft material.

It was a process that required a high level of concentration
so that not even the slightest membrane tear would occur,
but fortunately, the surgery was completed very stably.^^

Many people worry about eating
when surgery is done on both molars at the same time.
But don’t worry.
Even during the treatment period,
so that daily life is not disrupted,
we make a temporary denture that can be used immediately after surgery.

Of course, it may feel a little awkward at first,
and it is difficult to expect chewing ability equal to natural teeth,
but because it supports at least basic eating and aesthetics,
you can go through the recovery period without major inconvenience.
Taking care of the patient’s daily life as well
is also an important part of our dental treatment.^^
Five months of waiting,
finally regaining strong molars

Five months after placement, thanks to the careful management according to our clinic’s instructions,
the implant and jawbone had become very firmly integrated.

Now, to place the crown (prosthesis) on the implant,
we needed the process of taking an impression of the mouth.
At Seoul Bardi Dental Clinic,
we use an oral scanner that digitally and precisely reproduces the inside of the mouth.

Unlike in the past, there is no need to bite into cold, sticky impression material
and wait uncomfortably,
so this is one of the steps that many patients find very comfortable.^^

Based on the scanned data,
we completed a precise zirconia prosthesis that fits the patient perfectly
in our in-house dental lab.

Even though you sense that something is wrong with your tooth,
continuing to endure it with a temporary fix is probably due to
an undefined fear of dental treatment...
But using the tooth with only the outside stabilized,
without finding the cause,
can instead speed up damage to the jawbone
and become the reason for even larger treatment later.
If delaying treatment has left you feeling heavy-hearted,
you can let go of that burden a little now.
It’s okay not to decide on major surgery right away.
