Even after becoming adults, there are people who still use the baby teeth they had when they were young.
These are teeth that were left in place because permanent teeth were congenitally missing, so there was no choice.
The problem is that baby teeth have roots that are too short and weak to withstand the strong chewing force of an adult.

Eventually, when baby teeth reach their limit and become loose or inflamed,
they often quickly dissolve the surrounding gum bone as well.
Today, I will share a special case in which we overcame gum bone that had become as thin as paper due to weakened baby teeth,
and successfully placed implants.

A patient in their 20s who recently visited our dental clinic had been using baby teeth since the upper and lower first premolars on both sides
were congenitally missing.

However, due to the limitations of the baby teeth,
the upper right baby tooth (#55)
had already developed severe decay, and because the gum bone had melted away significantly, it was extracted.

The upper left baby tooth (#65) had only received root canal treatment
and was barely holding on without crown protection.

To place the implant, we checked the condition of the upper right gum bone with CT.

The remaining bone height was only about 1.1 mm.
In general, to place an implant safely,
at least about 8 to 10 mm of bone height is needed.
But a measurement of 1.1 mm means the bone is practically almost nonexistent,
so it can be considered extremely thin, like a sheet of paper.
Just above it is the maxillary sinus, an air-filled cavity,

so this was a very cautious situation with a high risk of damaging the membrane.
With the gum bone being this thin,
you would naturally worry whether an implant would even be possible.
I will now show you step by step how I safely resolved this difficult situation.
Sinus lift, which method is right for my condition?

When the bone height is severely insufficient like this,
a highly advanced procedure called a sinus lift must be performed.
There are two methods for sinus lift.
- Crestal approach
A method in which the sinus membrane is gently pushed upward through the hole made for implant placement,
and bone graft material is placed.

It is relatively simple and causes less pain,
but it is mainly used when a certain amount of basic bone height remains.
- Lateral approach
This is the method chosen when the bone is extremely, extremely thin, as in this case.
The sinus membrane is lifted by approaching through the side wall of the maxillary sinus,
and bone graft material is placed.

It requires a direct field of view,
and because the thin sinus membrane must not be torn,
it is considered a highly advanced procedure that must be handled with great precision.
Sinus bone graft through the lateral approach

Because the patient’s bone was only 1.1 mm thick, it was difficult to obtain even the minimum force needed to stabilize the implant.
So we decided to safely approach from the side using the lateral sinus lift,
place enough bone graft material,
and then place the implant.

Although it was a highly difficult surgery,
fortunately the sinus membrane was lifted very neatly,
and the bone graft was also carried out stably.^^
About 5 months after surgery, we checked numerically how well the implant had integrated with the bone.
Generally, a value of 70 or higher is considered to indicate safe stabilization.

Since the value was measured very high, at over 80,
we were able to confirm that the bone and implant had integrated well.
The implant tooth and the upper left baby tooth, which had only undergone root canal treatment,

were also firmly protected with a zirconia crown,
bringing the overall treatment to completion. ^^
