brushing it is difficult, so surprisingly many people
end up leaving it unattended.
As a result, without realizing it,

Today, based on a real case,
I’ll go over everything about the treatment process
for the backmost molar.^^
- Why does the backmost molar especially tend to decay?
If you lightly brush your backmost molar because the toothbrush can’t reach it, this is what happens.
Even if you try to put the toothbrush deep into your mouth,
the shape of the oral cavity makes it very hard for the brush head to reach the very end, right?

Because of that, food debris keeps accumulating
on the back of the tooth.
As this debris builds up,
a cavity starts on the back side surface of the tooth (distal surface).

This area is very close to the jawbone, so even if the cavity
progresses only a little,
it can quickly affect the nerve and the jawbone.
The problem is that this area is not even visible in the mirror,
so by the time a patient comes in because of pain,
root canal treatment or extraction is often already unavoidable ㅠㅠ

This is a case of a patient who recently visited the clinic with severe pain in the lower left molar.
When we looked inside the mouth, the lower left backmost molar (#37)
was already broken,

and the upper molar on the opposite side (#27)
had only the root left.

In fact, when we examined the X-ray and CT scans,
the situation was even more serious.

The lower molar (#37) already had decay that had
reached the nerve,
and especially the bone behind the tooth had already
deeply deteriorated from inflammation.

The upper molar also had inflammation spreading all the way to the root tip.

Treatment plan
Even for me, who is sincerely committed to saving natural teeth,
I wanted to save them as much as possible, but ㅜㅜ
Because the bone loss from inflammation was too extensive,
we unfortunately decided to proceed with extraction followed by implants.

Both teeth had severe bone loss,
so bone grafting was necessary.
For the lower molar (#37), we safely performed bone grafting
while considering the distance from the nerve canal,
but the real challenge was the upper molar (#27).
In the backmost upper molar area, there is an anatomical structure
called the "maxillary sinus," which is an empty cavity.

When placing an implant,
it is important to operate without damaging this maxillary sinus.
However, this patient had very thin bone,
and there was almost no foundation to support an implant....
In this case, bone grafting combined with a sinus lift procedure
is needed.
What is a sinus lift procedure?
If you lightly brush your backmost molar because the toothbrush can’t reach it, this is what happens.

This is a highly advanced procedure in which the very thin,
eggshell-like membrane of the maxillary sinus is lifted upward,

and then bone is artificially filled into that space.
Because the membrane can tear with even a slight mistake,
the medical team’s skill determines the success of the procedure!
Fortunately, the surgery was completed well without major issues.^^

After two months, we confirmed that the bone had solidly healed
and finished with a zirconia restoration.

Now, without worrying about throbbing pain,
the patient can eat freely with the backmost molar,
which was also a very rewarding case for me. ^^

"If it hurts, I’ll go in."
Even if it looks fine on the outside, inflammation that is
melting the bone may be progressing on the hidden side.
The most important thing is thorough brushing,
