Vertical lines on front teeth: Why the same crack can lead to completely different diagnoses

A thin vertical line on a front tooth
that you suddenly notice while looking in the mirror.
"Will my tooth just snap
while I’m eating someday?"

Once it catches your eye,
you keep looking in the mirror,
and it can be more upsetting than you’d expect.
Even if the crack in the front tooth looks the same,
the treatment method can be completely different for each patient.
What causes the vertical line
that suddenly appears on a front tooth,
and why can the diagnosis differ even when
patients come in with the same concern?
I’ll explain the reasons carefully
from the patient’s perspective through this treatment case.
- Why do vertical lines appear on front teeth?
A tiny crack in the tooth’s hardest outer layer, the enamel,
is called a tooth crack.

It can happen naturally from eating hard foods, clenching your teeth,
or simply due to aging.
Even if a crack with a fine vertical line does not look obvious at first,
it needs attention because continuous external force
can cause it to spread little by little.

"It doesn’t hurt,
so can’t you just fill the cracked part a little?"
That vertical line crack can feel like a thorn in your side...
Wouldn’t it be nice if you could simply remove just that area cleanly?
Because there is no pain right away, it may seem like you could just cover that area slightly with a material like resin...!
But in fact, simply removing the visible line
does not solve the fundamental problem the tooth has to withstand.
If treatment is done inappropriately while ignoring the cause,
the material may break or

it can lead to the crack spreading further inside the tooth.
So when deciding on a treatment plan,
it is not simply a matter of saying, “There’s a crack—let’s just remove the line.”
We have to consider the patient’s chewing force, eating habits, and bite relationship comprehensively.
Even if the crack looks the same,
why can the diagnosis differ so drastically?
I’ll explain through two cases.
Vertical line on a front tooth, cracked case <First case>
The first patient was a woman who came in saying the lines on her front teeth and canines looked unpleasant.

The canine area had already become discolored and looked dark.
The patient wanted laminate veneers that would be attached only lightly to the surface.

After examining her...
this patient had a bite close to an edge-to-edge bite.
What is an edge-to-edge bite?
Normally, the upper front teeth should slightly overlap the lower front teeth,

but this refers to a structure where the tips of the upper and lower front teeth
meet exactly edge to edge.

For people with this structure, the impact on the tips of the front teeth during meals is enormous.

Because the tooth edges were already heavily worn,
what would happen if a thin laminate veneer were attached?
There was a 99% chance it would break quickly.
In fact, when we see patients whose laminate veneers have broken,
there are many cases where treatment was carried out too aggressively without considering their bite relationship.

So, to prevent breakage and achieve aesthetics at the same time,
we proceeded with PFZ crowns.
What is PFZ?
The inside is made with a framework of zirconia,
which is as strong as metal,
and the outer surface is covered with natural-looking ceramic.
It is a prosthesis that offers both strength and beauty,
and is effective for front tooth treatment.

Since the patient’s profession made a neat impression important,
we completed the six upper front teeth in a balanced way,
including the cracked teeth,
so that the overall harmony would be improved.

Vertical line on a front tooth, cracked case <Second case>
The second case was a male patient who had very dark vertical lines on his front teeth.
The crack had formed from trauma after hitting something in the past,
and because it had not been managed afterward,
it had turned black along the line.

On the surface, it may look much more serious than the first case...
but for this patient, laminate veneers were actually possible.

This patient’s upper front teeth
normally covered the lower front teeth well,
and overall tooth wear was minimal.

In other words, it means the chewing force was not strong.
In such cases, there is no need to do a crown that shaves the tooth all the way around.
Laminate veneer method that attaches after minimally reducing only the outer surface of the tooth
By minimally shaping only the outer surface of the tooth,
simply attaching laminate veneers can be enough to solve the problem.

This is a case in which the bothersome black line was improved seamlessly with minimal reduction.
Even if the vertical lines on the front teeth look the same,
the correct treatment can be completely different depending on the patient’s chewing strength, eating habits, and bite structure.
Rather than always grinding down or covering the tooth,
the top priority is getting an accurate diagnosis of what method will last longest and be safest in your oral environment.
