Hello.
Honest treatment, easy-to-understand explanations!
This is Seoul Dia Dental Clinic near Magok Station, where smiles come to mind before worries.
Have you ever had this experience?
The cavity was deep, so you even had root canal treatment,
but a few months later, the throbbing comes back and you feel a sharp pain when chewing.
“Hmm… they said the nerve was completely removed, so why does it hurt again?”
At times like this, it may not just be sensitivity,
but a strong possibility that inflammation remains at the tip of the tooth root.
The treatment needed in that case is apicoectomy (root-end resection),
and although the name may be unfamiliar, the principle is simpler than you might think.
Let me explain step by step from here.
What is apicoectomy?

Magok-dong Dental Clinic_Inflammation removal at the root of the tooth
Apicoectomy, as the name suggests,
is a treatment that removes inflammation by cutting off the very tip of the tooth root (apex).
Most bacteria are removed through root canal treatment,
but the very deep area at the root tip may not be reached by instruments,
so bacteria can hide there and later cause problems again.
In such cases, the dentist approaches from outside the gum,
slightly removes only the inflamed root-end area, and removes the bacteria.
This is the essence of apicoectomy.
When apicoectomy is needed
Situations where apicoectomy may be needed
It is not a treatment everyone needs,
but if even one of the situations below applies, it may be worth checking.
- When pain continues even after root canal treatment

If pain remains when chewing or pressing even after root canal treatment is finished,
bacteria remaining at the root tip are likely the cause.
In that case, painkillers or antibiotics may temporarily calm it down,
but the pain soon returns.
In the end, the root-end inflammation itself must be removed for a real solution.
- When the gums swell or pus comes out

Magok-dong Dental Clinic_Gum pus pocket
If one side of the gums becomes swollen,
or if pressing on it feels like pus comes out with a small “pop,”
that is a sign that inflammation has spread to the apical area.
This is not just simple gum inflammation,
but may mean that bacteria from the tooth root tip have spread to the jawbone.
At this point, the source of the inflammation must be removed directly
to prevent pus from collecting again.
- When retreatment of the root canal is difficult

Magok-dong Dental Clinic_Abnormal root canal
Sometimes the root canal is so narrow or curved
that bacteria cannot be reached through the existing pathway.
In such cases, rather than repeating root canal treatment,
approaching from the outside through apicoectomy may be more effective.
- When the tooth is still too valuable to replace with an implant

Magok-dong Dental Clinic_When the tooth structure is otherwise normal
Many people say, “If it hurts, just pull it out; we can place an implant, right?”
but once a natural tooth is lost, that is the end of it.
Apicoectomy is a way to save a tooth that feels too valuable to extract for an implant,
and if the tooth structure is intact, there is no need to extract it;
it may be sufficient to remove only the inflammation and continue using the tooth.
If we summarize the cases where apicoectomy is needed,
-
Pain remains even after root canal treatment,
-
Pus develops in the gums,
-
It is difficult to resolve with retreatment of the root canal, and
-
It feels too valuable to extract the tooth,
then it is “the last treatment to save the tooth.”
