Because there isn’t enough bone height.
But when we take a CT scan,
there are cases where we think, “Oh? There’s more left than I expected?”
In such cases, it may be possible to place the implant without having to lift the upper sinus space.
What’s important is not intuition.
Rather than “I think it’ll work,”
we judge it numerically, like “There is 7 mm left right now.”
Implants are not placed by feel;
they are placed based on calculation.

“Implants are stronger when they’re long.”
It’s almost like the rule that a stake must be driven deep to keep it from wobbling.ㅎ
But these days, things are a little different.
If there is still a certain amount of jawbone remaining,
sometimes a short implant is enough
to secure stability.
Simply put,
it means that if the design is right, it can stand firmly.
If there is still a certain amount of jawbone left!
In that case, there are options for proceeding with the implant alone
without necessarily touching the sinus.

It is the method of placing the implant at a slight angle,
known as angled placement.
Rather than directly touching the sinus,
we adjust the angle to go in along the direction of the remaining bone.
Simply put,
it’s a strategy of using a side path
instead of forcing your way through a blocked road.
However, this is not possible just anytime.
The direction of biting force and balance with the surrounding teeth
must also be precisely calculated.
It is not about tilting by instinct,
but about calculating the angle.
This is where experience makes a difference.

Sinus lift surgery is not necessary in every case.
If there is enough bone,
it may be possible with a short implant,
and if the angle allows,
there are ways to place the implant while avoiding the sinus.
But if the conditions are not right
and you try to avoid it forcefully,
it can actually lead to more problems.
In the end, there is only one answer.
It’s not “Should I do it or not?”
It’s “What condition is my bone in right now?”
