and end up damaging the neighboring teeth.
Today, we’ll go one step further.
"I have a wisdom tooth, but it has never hurt.
Do I still need to have it removed?"
This is a very important question.
But here’s the thing.
Just because it doesn’t hurt doesn’t mean it’s okay.
In fact, wisdom teeth that don’t hurt can sometimes create problems more quietly,
and more deeply.
Today, I’ll go through why asymptomatic wisdom teeth can be more dangerous,
one by one.
Why doesn’t it hurt?
There are two main reasons
why a wisdom tooth may not hurt.
One is when it really is fine.
As I mentioned in Part 1,
this is when it has erupted straight, there is enough space, and there is no inflammation.
But there is another case.
When it is completely buried under the gums.

A completely impacted wisdom tooth
has not come out through the gums, so it doesn’t get irritated.
That’s why it doesn’t hurt.
But problems may be quietly building up inside.

- It quietly dissolves the root of the neighboring tooth
I mentioned this briefly in Part 3.
When a completely impacted wisdom tooth is pressed up against
the root of the adjacent tooth, that pressure slowly causes the root to be resorbed.
There is no pain, and nothing is visible from the outside,
and everything may seem fine, but the root is quietly dissolving.
By the time this is found,
it is often already quite advanced.
- A cyst can develop
The word cyst may sound unfamiliar.
Around an impacted wisdom tooth,
there is leftover follicular tissue that once surrounded the tooth.
Over time, this tissue can sometimes turn into a cyst filled with fluid.
A cyst slowly grows, dissolves the surrounding bone,
presses on nerves, and in severe cases can even lead to a jaw fracture.
The scary part is that a cyst usually doesn’t hurt in its early stages either.
Symptoms often appear only after it has grown quite large.
- It can suddenly flare up one day
A wisdom tooth that has been quietly asymptomatic
may suddenly come with severe pain
as inflammation flares up all at once.
When your immune system is weakened,
when you are under a lot of stress, or when fatigue has built up severely.
The inflammation that has been quietly accumulating can suddenly break out.
It happens more often than you might think that this occurs right before an exam,
just before an important event,
or during an overseas trip.ㅎㅎ
- It presses on nearby structures inside the bone
Around the lower wisdom tooth,
there is an important nerve called the inferior alveolar nerve.
If an impacted wisdom tooth is positioned close to this nerve,
it can press on it over time.
At first, you may not notice it,
but later it can appear as numbness or tingling around the tip of the chin
or the lips.

So does it have to be removed no matter what?
Not necessarily.
Just because it is asymptomatic does not mean it must be removed right away.
But what matters is knowing the current condition accurately.
Even if there is no problem right now,
if we check with an X-ray,
we can figure out in advance:
- how close it is to the root of the neighboring tooth
- whether a cyst is developing
- how far it is from the nerve
- whether there is a chance of future problems
Based on that result, we can decide whether it is better to remove it now,
or whether it is okay to keep watching it.
Putting off an evaluation just because it doesn’t hurt
may be the riskiest choice.

Now, let’s briefly summarize today’s post.
- There are two reasons it may not hurt → either it is truly fine or it is impacted
- An impacted wisdom tooth quietly dissolves the root of the neighboring tooth
- If a cyst develops, it can dissolve bone and lead to a jaw fracture
- Inflammation can suddenly flare up one day
- Even if it is asymptomatic, checking the current condition with an X-ray comes first!
Important point!
It’s not "It doesn’t hurt, so it must be fine,"
but rather "It doesn’t hurt, so you should check it now."
