"My implant is loose!"

When patients contact a dental clinic with these concerns, most of them speak in a truly startled voice.

But here is the thing.

A loose implant does not always mean that revision surgery is necessary.

Depending on the cause, some cases can be resolved easily, while others require a detailed examination.

First, know where the mobility is occurring

When you feel that your implant is loose, there are actually two possibilities.

1. The prosthesis (crown) is loose
2. The implant itself (fixture) is loose

The causes, seriousness, and solutions for these two situations are completely different.

Case 1. The prosthesis is loose

This is the most common case.

An implant crown is attached either by securing it with a screw or by bonding it with cement.

Over time, the screw may loosen or the cement may weaken, causing only the crown to become loose.

Fortunately, this can be resolved relatively easily.

When you visit the dental clinic, it can usually be addressed quickly by tightening the screw again or rebonding the crown.

In most cases, the implant root (fixture) itself is still in good condition.

• Frequently chewing hard foods

• Grinding your teeth

• The screw naturally loosening over time

• The screw not being tightened sufficiently when the prosthesis was attached

Case 2. The implant itself is loose

This case requires more careful evaluation.

An implant is firmly placed in the bone and secured through a process called osseointegration.

If the implant itself is loose, it may be a sign that there is a problem with osseointegration.

There are various possible causes.

1. Peri-implantitis

If a bacterial infection causes inflammation around the implant and the inflammation spreads to the bone, the bone supporting the implant may be lost, causing mobility.

2. Excessive occlusal force

If excessive force is continuously placed on the implant due to teeth grinding or clenching habits, osseointegration may become weakened.

3. Failed osseointegration

This occurs when osseointegration was not sufficiently achieved from the beginning.

Overall health, smoking, and diabetes control can have an effect.

4. Trauma

The connection between the implant and the bone may also be damaged by a strong impact or accident.

What should you do if the implant itself is loose?

In this case, the most important thing is to visit a dental clinic as soon as possible.

The longer it is left untreated, the more surrounding bone loss may progress, and the fewer treatment options may remain.

Confirmation through an examination

X-rays and detailed examinations are used to check:

• The extent of bone loss

• Whether inflammation is present

• Whether the implant itself is damaged

The treatment approach depends on the condition

If the mobility is mild and at an early stage, it may be possible to preserve the implant through inflammation treatment and management.

However, if osseointegration has been severely compromised or substantial bone loss around the implant has occurred, the implant may need to be removed, followed by consideration of replacement after the bone has recovered.

How to prevent implant mobility

1. Be sure to have regular checkups

Implants require even more thorough care than natural teeth.

Because they do not have nerves, problems may often be noticed only after pain develops later.

It is important to have the gum and bone condition around the implant checked once every six months.

2. Prevent peri-implantitis

Please carefully clean around the implant using a toothbrush and dental floss, and use a water flosser if necessary.

3. Use a device if you grind your teeth

It is recommended to use a night guard along with other care to prevent excessive force from placing continuous strain on the implant and prosthesis.

4. Visit the clinic right away if you notice unusual signs

Do not take signs such as even a slight feeling of looseness, discomfort when chewing, or bleeding or swelling of the gums lightly.

The earlier a problem is found, the simpler the treatment can be.

The key is to have it checked quickly

You do not need to panic immediately just because your implant feels loose.

If only the crown is loose, the issue can often be resolved easily.

However, in any case, it is difficult to make an accurate judgment until the area has been examined directly.

Rather than putting it off for several days while thinking, "It will probably be fine," please visit the clinic promptly if you feel any looseness.
