Hello.
This is Mutoel Plastic Surgery Clinic.
When you get older, what is the first change you notice?
Patients who visit our clinic mainly mention the forehead and the area around the eyebrows.
As the years go by, the eyebrows gradually droop along with the skin, and as forehead wrinkles deepen, the face can look tired and heavy.
For mild forehead wrinkles, simple procedures such as Botox can be enough to improve them, but when the brow line itself sags, the outer corners of the eyes look lower and the surrounding area feels heavy, it is difficult to improve with a simple procedure.
At times like this, the surgery to consider is the "endoscopic brow lift."
Beyond a simple cosmetic purpose, it is an attractive option that can also be expected to provide positive functional improvement.
Today, let’s take a closer look at it one by one.

What is an endoscopic brow lift?
It is a surgery in which a small incision is made within the scalp, an endoscopic device is inserted, the forehead muscles and the soft tissue around the eyebrows are dissected, and then they are pulled upward and fixed in place.
Unlike traditional incision-based lifting procedures, the incision area is not large, so there is clearly less concern about scarring, and recovery is relatively faster.
However, the biggest advantage of this surgery is that it can comprehensively improve the eyebrows, forehead, and eye area.
Rather than simply pulling up the skin, it also addresses and improves the frontalis muscle and the glabellar muscles that deeply affect facial expressions, such as the corrugator and procerus muscles, allowing the overall impression to become softer and more refreshed than before.

What is the goal of the improvement?
If the surgery is performed, the eyebrows first move up naturally from their original position.
When this happens, drooping-looking eyelids improve, and the eyes appear much clearer.
Wrinkles such as vertical forehead lines and frown lines between the eyebrows also improve at the same time, and in many cases even wrinkles around the eyes are softened.
When these changes come together, the result goes beyond simply looking younger and creates a more comfortable and lively impression.
This surgery can be especially helpful for people who often hear things like, "You look angry" or "Do you look tired?"

How is the surgery performed?
An endoscopic brow lift is usually performed under local anesthesia with sedation, or under general anesthesia if needed.
About 4 to 5 small incisions of around 1 to 2 cm are made in the back part of the scalp, and the endoscope is inserted through them to dissect the eyebrow and forehead muscles.
If necessary during this process, muscles that cause wrinkles, such as the corrugator muscle, are removed or weakened, the eyebrows are lifted upward, and they are firmly fixed using genuine Endotine.
Because the surgery is performed while directly confirming the structure with precision through the endoscope, the risk of bleeding and complications is low, and the long-standing problems seen with traditional incision methods, such as large scars and sensory changes, are also reduced.
The operation takes about one hour, and it is a relatively low-burden procedure to the point that same-day discharge is possible.

Recovery and return to daily life
Pain after surgery is generally minimal, and swelling and bruising vary by person, but in typical cases they gradually subside within 1 to 2 weeks.
The incision sites are hidden inside the hair, so there is almost no concern about visible scars. However, if it is performed together with a semi-permanent hairline procedure, hairline refinement and scar coverage can be done at the same time to match the lifted skin, making the combined approach more effective.
Stitches are removed about 7 to 10 days after surgery, and returning to work or daily life is usually possible in about a week.
There may be temporary numbness in the surgical area or a slight pulling sensation, but in most cases these improve within a few months, and the final results settle naturally over 1 to 2 months.

Who is it recommended for?
When people are considering cosmetic surgery because their eyelids are drooping, in many cases the eyelid skin is not significantly stretched; instead, the eyebrows themselves have descended.
In such cases, rather than a procedure like upper blepharoplasty, an endoscopic brow lift may be a more attractive option.
In fact, among these patients, many initially consider double eyelid surgery or eyelid surgery, but after an accurate diagnosis, they change their plan and achieve better results.
Second, for those whose face looks angry, whose expression seems fixed in a frown, or whose glabellar muscles are excessively developed so that the wrinkles are deep, the eyebrows are pulled downward, and the face looks strong, surgery can soften the glabella and refine the brow/hairline, making the overall impression much softer.
Lastly, for middle-aged adults in their 40s to 60s who are bothered by wrinkles around the eyes and forehead: when you want to improve wrinkles, excessive muscle tension, and a collapsed skeletal structure that Botox or fillers cannot fully address, this is a recommended surgery because it can resolve structural issues.

Why combining it with a semi-permanent hairline procedure is beneficial
Because the surgery involves dissecting the scalp and lifting the eyebrows and forehead upward, the hairline may appear higher after the procedure.
In particular, people with a wide forehead may worry that their forehead will look even wider.
If you combine it with a semi-permanent procedure at this time, it can visually narrow the width of the forehead and create a more natural and refined impression.
In addition, the overall upper-face line becomes more organized, facial proportions look more attractive, and the youthful effect is layered on top, making it possible to achieve both structural improvement through surgery and finishing touches through the procedure.

In closing
As with any other surgery, side effects can occur.
Temporary sensory loss, a pulling sensation, and undercorrection are among the possible side effects.
Therefore, before surgery, it is most important to consult with an experienced board-certified plastic surgeon who can accurately analyze and plan according to the patient’s skull structure, the position of the forehead muscles, and eyebrow height.
An endoscopic brow lift is closer to "improving facial expression through structural change" than simply "pulling up the forehead."
It can be a good option to consider for improving the area around the eyes, which is a key part of youthful plastic surgery, and if you are looking for a way to address and improve sagging in the forehead and brow line, along with wrinkles around the eyes, that Botox or fillers could not sufficiently resolve, it may be the answer.
Of course, the right answer depends on each patient’s facial structure and goals, so if you want a solution that fits you exactly, we recommend making sure to adjust it through consultation with a specialist.
