The area around the eyes is where signs of fatigue show up the fastest on the face.
Being told, “You look tired” or “You look older”
often starts with changes under the eyes.
In particular, when under-eye fat protrudes prominently
or the tear trough beneath it becomes deeper,
it can create shadows or a dark-circle-like appearance,
making you look even more tired than your actual age.
At this point, simply filling in the hollow area with a procedure alone makes it difficult to achieve a fundamental improvement,
and various efforts to change the structure of the under-eye area are needed.
So in this post,
we will introduce under-eye fat repositioning and lower blepharoplasty
in detail through cases.

What is under-eye fat repositioning?
"Under-eye fat repositioning" is not simply a surgery that removes fat.
It does remove some of the fat that bulges beneath the eyes,
but more importantly, it moves and repositions the fat into the hollow tear trough or upper cheek area to smooth the under-eye contour.
Because the incision is made through the conjunctiva inside the eyelid,
there is no visible external scar,
and the procedure reduces under-eye bulging while improving both hollowness and shadowing at the same time.
As the word "repositioning" suggests,
it is easier to understand this as a surgery that corrects the position of the fat.
What is lower blepharoplasty?
Lower blepharoplasty is a surgery that improves loosened under-eye skin
and bulging fat together, helping refine an appearance
that looks tired or older.
As aging progresses, the skin under the eyes loses elasticity and sags.
In this process, the fat inside may be pushed outward,
or fine lines may layer on top of each other,
making the eye area look heavy.
Lower blepharoplasty is intended to fundamentally address these structural changes.
It is not simply a matter of cutting away skin,
and it is important to plan the surgery by comprehensively considering the condition of the under-eye fat,
the degree of skin sagging,
the direction of the outer corner of the eye,
and the balance of the eye shape.
If necessary, removing unnecessary fat and excising only a moderate amount of skin
is the key to creating a natural-looking under-eye area.
If you want to improve under-eye bulging, skin sagging,
and fine lines at the same time,
or if under-eye fat repositioning alone is expected to have limitations in effect,
lower blepharoplasty can be a complementary option.

Who is it recommended for?
Here, with real cases, we will introduce when these surgeries are recommended.
First, under-eye fat repositioning is the surgery we primarily recommend for people who have been bothered by prominent under-eyes since a young age.
When looking at the faces of such people,
most tend to have large, round eyes
or a protruding eye structure where the eyeballs sit relatively forward.
Orbital fat, which exists to protect the eyes,
is influenced by the shape and position of the eyeballs.
When the eyeballs are large or protruding, this fat is also pushed forward,
which makes the under-eye area more likely to appear bulging.
In this type of under-eye area, the issue is often the position and distribution of fat rather than skin sagging,
so instead of lower blepharoplasty that removes skin,
under-eye fat repositioning through the conjunctiva is prioritized.
Also, many people with protruding eyeballs have a facial shape with insufficient midface volume,
so additional fat grafting is often recommended.
By replenishing the upper cheek area connected to the under-eye region,
and, if necessary, also filling the nasolabial fold area,
we perform customized surgery that considers the overall balance of the face through fat grafting.

By replenishing the previously insufficient midface volume, we enhanced a more youthful look.

This patient originally had good midface volume,
but wanted an even more youthful impression, so fat grafting was added.
Then is under-eye fat repositioning for younger people,
and lower blepharoplasty for older people?
Not necessarily.
A more important criterion than age
is what concern the person feels is most uncomfortable.
If the main concern is discomfort when looking downward, such as while reading a book,
because of bulging under the eyes, then the bulge needs to be addressed.
In such cases, rather than lower blepharoplasty,
a surgical plan centered on under-eye fat repositioning with additional fat grafting can lead to satisfying results.
However, because the skin is not excised,
we proceed only after fully explaining that natural wrinkles may remain due to movement around the eyes after surgery.
Conversely, when analyzing the midface skeleton and soft tissue,
if the volume is sufficient,
fat grafting is not necessarily needed.
For these people, because the loosened skin also needs improvement along with under-eye fat repositioning,
we recommend lower blepharoplasty including skin excision and lateral canthal fixation.

An upper eyelid lift was performed, and because the midface volume was good, lower blepharoplasty was performed.
In the photo above, for the female patient, we performed the safer and faster-recovery under-eye fat repositioning through the conjunctiva,
and at the same time made a minimal incision near the eye area to remove excess skin and fat,
then completed the procedure with lateral canthal fixation.
By making a minimal incision on the outer side to minimize eyelid scarring and distortion,
and firmly suspending and fixing the lower eyelid to the orbital bone,
we can see that the irregularities under the eyes improve significantly even without fat grafting.


Recovery and aftercare
After surgery, although there are individual differences,
bruising and swelling usually subside gradually over about one week.
While the swelling goes down and the wound heals,
you may feel pulling or stiffness under the eyes,
but this usually improves gradually after one month,
so there is no need to worry.
Avoid rubbing your eyes hard,
drinking alcohol,
saunas,
and strong heat exposure for at least two weeks after surgery.
